Aim: Psychosocial distress can act as a barrier to diabetes self-care management and thus compromise diabetes control. Yet in Ghana, healthcare centres mainly focus on the medical aspect of diabetes to the neglect of psychosocial care. This study determined the relationship amongst psychosocial distress, clinical variables, and selfmanagement activities associated with type 2 diabetes management. Method: Questionnaires were administered to 162 patients from four hospitals in Accra, Ghana, to assess psychosocial distress (e.g. diabetes distress), clinical variables (e.g. glycaemic control), and self-management activities (e.g. medication intake) related to diabetes. In assessing diabetes distress, the use of the PAID allowed evaluation of broader range of emotional concerns (diabetes-related emotional distress), while the DDS allowed evaluation of factors more closely related to diabetes self-management (diabetes distress). Results: Diabetes-related emotional distress, diabetes distress and depressive symptoms were reciprocally positively correlated, while non-supportive family behaviour correlated negatively with these psychological variables. Diabetes-related emotional distress correlated positively with systolic and diastolic blood pressure, and correlated negatively with exercise regimen. On the other hand, diabetes distress correlated negatively with dietary and exercise regimen and correlated positively with glycaemic levels, while depressive symptoms correlated positively with glycaemic levels, diabetes complication and systolic blood pressure. Contrary to the literature, non-supportive family behaviour correlated positively with diet, exercise and medication regimen. Conclusion: The positive association of psychological variables with glycaemic levels and blood pressure levels, and the positive association of non-supportive family behaviour with self-management activities suggests the need for psychosocial care to be incorporate in the management of type 2 diabetes in Ghana. Patients can be screened for diabetes-related distress and symptoms of depression and provided psychosocial care where necessary.

Correlation between glycated hemoglobin and venous blood sugar in diabetic patients monitored in Abidjan

The aim of this study was to determine the correlation between glycated hemoglobin and blood sugar levels in diabetic subjects carried out in Abidjan. This cross-sectional study included 100 patients with diabetes monitored, for three months, for whom glycated blood glucose and hemoglobin were performed, this after informed consent of the patients. Pearson and Spearman correlation tests were used, at the 5% threshold. The patients with normal HbA1C and normal blood glucose accounted for 55.34 and 32%, respectively. A sedentary lifestyle and body mass index > 25 kg/m² were associated with a significant increase in the risk of increased blood glucose and HbA1C. The presence of a complication was associated with a 3.06-fold higher risk of high glycated hemoglobin (p= 0.0073), while blood glucose was not significantly associated with the onset of complications. Glycated hemoglobin was significantly correlated with blood glucose with a correlation coefficient of 0.4412 (p 0.0001). In multivariate analysis, hyperglycemia was significantly associated with alcohol consumption and noncompliance with antidiabetic treatment. Poor compliance was significantly associated with increased glycated hemoglobin (>7%) and tobacco consumption. The glycated hemoglobin was highly correlated with blood sugar, and was found to be a better predictor of diabetes complications than glycemia.

Aim: To describe the patient's experience of family function and its importance in diabetes-related self-management. Background: Many patients fail to reach the targeted glycemic level due to low self-management adherence. Knowledge is needed regarding the impact of family function on diabetes selfmanagement. Design: A qualitative descriptive design. Methods: A purposive sample of 20 patients with type 2 diabetes. Data were collected in March-June 2017 via audio recorded semi-structured interviews, field notes and Eco-maps. Analyzed using thematic framework matrix and thematic analysis. Results: Four themes were identified: (1) Downplaying disease. The disease was trivialized creating a barrier to family involvement; (2) Second guessing. When diabetes was not discussed, patient and family made their own assumptions; (3) Going it alone. The participants preferred sole disease responsibility to maintain usual family life; (4) No regrets. The participants managed their disease with medications only to maintain family cohesion and "the good life". Conclusion: The participants in our study downplayed the consequences of type 2 diabetes and chose to control their disease medically rather than by lifestyle changes. They renounced family involvement to maintain their lifestyle and promote family cohesion.
