Management of psoriasis is a challenge to the treating physician. The chronic inflammatory state of psoriasis with exacerbations and remissions necessitate “on-and-off” treatment schedules. The safety profiles of drugs and tolerability issues for patients are important factors to be considered during treatment. Various biological agents targeting T-cells and the inflammatory cytokines are available for systemic treatment of psoriasis. However, major causes of concern while using these drugs are risk of susceptibility to infection and development of anti-drug antibodies, which will affect the pharmacokinetic properties, efficacy, and safety profile of the drug. Itolizumab, a humanized anti-CD6 monoclonal antibody, is a new molecule that acts by immunomodulating the CD6 molecule. CD6 is a co-stimulatory molecule required for optimal T-cell stimulation by the antigen-presenting cells. This step is crucial in T-cell proliferation to form Th1 and Th17 cells, which play a major role in the pathogenesis of psoriasis. This article deals with the properties of Itolizumab and its role in the treatment of psoriasis. Based on the available published data, Itolizumab seems to have a better adverse effects profile and at the same time comparatively less efficacy when compared to other biological agents available for treating psoriasis. Larger studies with longer duration are required to clearly depict the long-term side effects profile.

Background:Psoriasis is a chronic, immune-mediated skin disease with unknown etiology, with an epidermal turnover time of <10 days compared to a normal turnover time of 4-8 weeks. This epidermal hyperproliferation accounts for many of the metabolic abnormalities including alteration in the serum levels of proteins and some trace elements.Aim:The aim was to detect any statistically significant difference in the serum levels of zinc, copper, albumin, globulin and alkaline phosphatase between psoriasis patients and healthy controls.Materials and Methods:Hundred cases of psoriasis and 100 age and sex matched controls were enrolled in a hospital based case-control study. The serum levels of zinc, copper, albumin, globulin and alkaline phosphatase were calculated and compared among the cases and controls and evaluated statistically.Results:Serum zinc levels were significantly low in the psoriasis group as compared with controls (mean 80.028 μg/dl vs. 109.179 μg/dl, P < 0.0001). Serum copper levels were significantly raised among cases as compared with controls (mean 167.317 μg/dl vs. 133.884 μg/dl P < 0.0001). Serum albumin levels were significantly decreased (3.762 g/dl vs. 4.103 g/dl, P < 0.001), whereas serum globulin levels were raised (3.296 g/dl vs. 2.596 g/dl, P = 0.0014) among cases as compared with controls, respectively. Serum alkaline phosphatase levels were comparable between the two groups.Conclusion:The results of this study show significant alterations in the serum levels of copper, zinc, albumin, and globulin in psoriatic patients. This paper aims at highlighting the possible role of trace metals copper and zinc in the aetiopathogenesis of psoriasis and also provides a proposed interplay of factors involved in the pathogenesis of psoriasis.

Psoriasis is a chronic inflammatory disease mediated by the immune system with a prevalence of 2-3% in the world population whose manifestations are found in the skin and other organs. Data on the prevalence of psoriasis is still diverse in several hospitals in Indonesia. Multifactors play a role in the incidence of psoriasis both genetic and environmental. Psoriasis lesions occur due to an imbalance between proliferation and differentiation of keratinocytes. Vitamin D has a role in regulating the balance of the process. Vitamin D is a fat soluble vitamin that can suppress the inflammatory process in psoriasis lesions.This study aimed to determine the frequency distribution of age, sex and levels of 25-Hydroxyvitamin D in blood plasma 44 psoriasis patients.Kadar vitamin D tersebut diukur menggunakan metode ELISA (DBC). The results obtained were the average age of 42-year-old psoriasis patients with the youngest age of 18 years and the oldest at 77 years. Female sex was 56.8% and men were 43.2%.The highest vitamin D level was 59.1 ng / ml and the lowest was 7.3 ng / ml so that it averaged 25.92 ng /ml. Analysis of vitamin D levels produced suficiency (31.8%), insufficiency (31.8%) and deficiency (36.4%).It can be concluded that vitamin D levels in psoriasis patients in Medan city 68.2% have less categories (<29 ng / ml) and most deficiencies at 48-57 years old.
