Vestibular schwannomas and meningiomas are the most common lesions of the cerebellopontine angle (CPA), accounting for approximately 85-90% of the tumors seen in this location 1 . Lipomas are rare at this topography, representing about 0.15% of the CPA lesions 2,3 . These tumors are maldevelopmental masses that arise from abnormal differentiation of the meninx primitiva 1,4,5 . Clinically, CPA lipomas can cause slowly progressive neurological symptoms and signs affecting cranial nerves or brain stem 3,[6][7][8] . Because these lesions usually are strongly attached to the surrounding structures, any surgical attempts of complete resection can result in neural or vascular damage, reinforcing the importance of the pre-operative imaging diagnosis [1][2][3]7,9,10 . Although the CT findings of CPA lipomas can be typical, the magnetic resonance (MR) imaging, especially the fat suppression sequences, had improved the identification of these lesions.We aimed to report two patients with a CPA lipoma, emphasizing the MR imaging findings. CaSeCase 1 A 13-year-old female patient was evaluated due to a 1-year history of headache and hearing loss. The physical examination was unremarkable. The audiometric evaluation demonstrated a discrete sensorineural hearing loss on the right side. The CT scan revealed a markedly hypodense non-enhancing mass in the right CPA. The MR imaging showed a lesion measuring 2.1 × 2.0 × 1.7 cm in the right CPA cistern. The mass was hyperintense on T1-weighted images and isointense with hypointense halo (chemical-shift) on T2-weighted images, with very low signal on T1-weighted images with fat suppression (Figs 1 and 2). RESUMO -Apesar dos lipomas serem os tumores que mais comumente ocorrem no corpo caloso (CC), sua incidência na população é rara. Relatamos caso de menino, com 5 anos de idade e história de atraso no desenvolvimento psicomotor associado a dificuldade na marcha secundária a hipotonia generalizada. Na investigação, a ressonância magnética mostrou presença de lipoma curvilinear do CC, associado a hipertrofia do CC. Na literatura, pacientes com lipoma do CC possuem agenesia ou hipotrofia do CC, sendo que neste caso observamos pela primeira vez uma hipertrofia do CC. Tecemos considerações embriológicas, genéticas, clínicas, radiográficas e terapêuticas sobre pacientes com lipoma do CC relacionando-as à descrição deste caso. PALAVRAS-CHAVE: corpo caloso, hipertrofia, lipoma.Lipoma of the corpus callosum associated with the hypertrophy of the corpus callosum: case report ABSTRACT -Despite the lipomas are the tumors that more commonly occur in the corpus callosum (CC), its incidence in the population is not common. We report on a 5-year old boy, with history of retardation in the psychomotor development and disturbs in the gait, secondary to generalized hypotony. Magnetic resonance imaging showed a curvilinear lipoma of the CC related to its hypertrophy. In the literature patients with CC lipoma usually have agenesis or hypotrophy of the CC, but in the reported case we have seen, by the first time, a hypertrophy of the CC. We made embryologic, genetic, clinical, radiographic and therapeutic considerations about the patients that have CC lipoma comparing to findings in the case we report.KEY WORDS: corpus callosum, hypertrophy, lipoma.Lipomas são os tumores mais comuns associados ao corpo caloso (CC), entretanto sua incidência é rara. A história clínica e neurológica podem ser normais ou incluir crises convulsivas e déficite cognitivo. A ressonância magnética (RM) é o método de imagem mais preciso para o diagnóstico, sendo reconhecidas diferentes variedades de tumor 1,2 . O lipoma do CC está relacionado com agenesia ou hipotrofia do CC, mas a associação com hipertrofia, não tem sido relatada na literatura até o presente momento. Relatamos um caso no qual essa associação foi observada.
