Background: Even after the description of papuloerythroderma of Ofuji (PEO) in 1984, little is known about this clinical entity. Objective:To report on 2 new cases of PEO and review of the worldwide literature on this topic. Methods: Article citations were searched on several biomedical search engines (PubMed, EMBASE, SCOPUS, Google Scholar). Papers were retrieved either online or in print. Results: A grand total of 170 PEO cases were identified. Most patients were older than 55 years and of Asian or white descent, with an overall male/female ratio of 4.0. Itch and the deck-chair sign were observed in all patients. Peripheral eosinophilia, lymphocytopenia and increased serum IgE were common findings. Histopathology mostly showed aspecific inflammation, while 17 showed histological features of cutaneous T-cell lymphoma (CTCL). Atopy, malignancies, infections and drugs were rarely linked to PEO. Conclusion: PEO represents a rather monomorphous entity both clinically and, with the remarkable exception of CTCL, also histologically. Nonetheless, no causative factor could be identified in the vast majority of cases. An etiological classification and diagnostic criteria are proposed in the attempt to contribute framing this puzzling clinical entity.

ÖzetÇok sayıda iç organ kanseri deride pruritus (kaşıntı) ve eritrodermi ile giden tablolara yol açabilir. Bu tablolar iç organ kanserinin erken döneminde görülebilir veya onun aylar öncesinden ilk habercileri olabilir. Diğer yandan kaşıntı ve eritrodermi paraneoplastik hastalıklar için özgül semptomlar değildir; çok sayıda farklı nedenlerle ortaya çıkabilirler. Bu derlemede iç organ kanserlerine bağlı kaşıntı (paraneoplastik kaşıntı) ve eritrodermi (paraneoplastik eritrodermi)'nin epidemiyolojik ve klinik özellikleriyle birlikte nedenleri ve tanısal ipuçları anlatılmıştır. Paraneoplastik kaşıntı sıklıkla yaygın, inatçı ve kronik bir kaşıntıdır. Geceleri daha belirgin olabilir. Hastalarda kaşıntıya ikincil olarak ekskoryasyon, hiperpigmentasyon, likenifikasyon ve prurigo nodülleri gelişebilir. Paraneoplastik kaşıntılı olguların büyük bölümünde Hodgkin lenfoma başta olmak üzere diğer lenfomalar, lösemiler ve polisitemia vera saptanır. Paraneoplastik eritrodermi olağan eritrodermiye göre tedaviye daha dirençlidir. Ek olarak belirgin kilo kaybı ve halsizlik bulunabilir. İnce bir skuamlanmayla birlikte sıklıkla hiperpigmentasyon da (melanoeritrodermi) eşlik eder. Eritroderminin nadir nedenlerinden olan Ofuji'nin papüloeritrodermisinde şiddetli kaşıntı ile birlikte yaygın yerleşimli, eritemli ve üzeri düz papüller saptanır. Derinin T hücreli lenfoması ve özellikle de onun lösemik varyantı olan Sezary sendromu paraneoplastik eritroderminin en sık nedenidir. Paraneoplastik kaşıntı ve eritroderminin araştırılmasında ilk adım, ayrıntılı bir öykü alımı, dikkatli bir dermatolojik muayene ve lenf nodu muayenesini de içeren sistemik fizik muayene olmalıdır. Elde edilen bulguların ışığında ve öncelikle lenfoproliferatif hastalıklar üzerinde yoğunlaşılarak daha ileri tetkiklere başvurulmalıdır. Sonuçların normal sınırlarda bulunması durumunda bu tetkiklerin belirli aralıklarla tekrarlanmasında yarar vardır. These symptomes can occur in the early stages of internal malignancies or precede them by months. On the other hand, these symptoms can also be caused by various other common conditions; therefore they are not specific for paraneoplastic diseases. This review overviews the current state of knowledge regarding paraneoplastic pruritus and paraneoplastic erythroderma, and highlights the association between these conditions and internal malignancies together with their epidemiological, clinical and diagnostic features. Paraneoplastic pruritus is often generalized with intractable and chronic course. Nocturnal pruritus can be a significant problem for these patients. Patients may present with secondary skin changes including excoriations, hyperpigmentation, lichenification and prurigo nodules.
