Pancreaticoduodenectomy for the Management of Pancreatic or Duodenal Metastases from Primary Sarcoma

Abstract: To our knowledge, this is the largest series of patients with pancreatic metastases of sarcoma treated with surgical resection. Despite optimal resection with R0 margins, in the absence of effective adjuvant systemic therapies, the benefit of such an approach in metastatic disease remains unclear.

“…The cat with the pleomorphic sarcoma in our study had increased SAA values of an unknown cause and the pathogenesis of the tumor remained unclear. In human medicine, recurrence of metastatic sarcomas in the pancreas is common and a median overall survival time of 21 months [17] as well as moderate complication rates [18] have been described. The cats in our study were euthanized during surgery because of the poor prognosis.…”

“…There is one case report of a feline primary pancreatic carcinosarcoma with metastases in uterus, omentum, and diaphragm [16]. Mesenchymal pancreatic tumors in humans are reported to have a poor prognosis with a median overall survival time of 21 months [17,18]. Visceral hemangiosarcomas are a common neoplasm in dogs, especially German Shepherds, but are uncommon in cats [19].…”

“…In a tertiary-referral hospital for soft-tissue sarcoma with 6744 new cases of soft-tissue sarcoma, 7 underwent duodenopancreatectomy for sarcoma metastases to the pancreatic head. The median survival was 21 months (range: 4 days to 86 months) [117].…”

“…Surgery has been performed for both primary and metastatic sarcoma arising in the pancreas and duodenum[ 195 , 196 ]. The largest case series describes seven out of 17 existing reports of resection for pancreatic metastases, with one postoperative death and recurrence in all other six cases despite margin-free resection in four cases, with a median survival of 21 mo[ 195 ]. The benefits of resection therefore remain unclear.…”
