Malignant biliary obstruction due to metastatic non-hepato-pancreato-biliary cancer

Abstract: Malignant biliary obstruction generally results from primary malignancies of the pancreatic head, bile duct, gallbladder, liver, and ampulla of Vater. Metastatic lesions from other primaries to these organs or nearby lymph nodes are rarer causes of biliary obstruction. The most common primaries include renal cancer, lung cancer, gastric cancer, colorectal cancer, breast cancer, lymphoma, and melanoma. They may be difficult to differentiate from primary hepato-pancreato-biliary cancer based on imaging studies, … Show more

“…In addition to gastric cancer, malignant biliary obstruction can also develop as a result of metastasis from distant cancers, such as colorectal cancer, renal cancer, lung cancer, breast cancer, lymphoma, and malignant melanoma [ 15 ]. Renal cell carcinoma appears to be the most common primary tumor to cause secondary pancreatic tumors [ 16 ].…”

“…Cancer of the head of the pancreas is often associated with biliary obstruction leading to obstructive jaundice. High concentrations of bile acid in the liver cell predispose them to hypoxia and mitigate the effect of liver ischemia [ 55 ]. Normally, pancreatoduodenectomy is associated with 13 to 35% of morbidity due to pancreatic leak.…”

“…Malignant biliary obstruction is typically caused by primary malignancies of the pancreatic head, bile duct, gallbladder, liver, and Vater’s ampulla [ 122 ]. Lesions that have spread from other primary sites to these organs or nearby lymph nodes are a less common cause of biliary obstruction.…”
