Palliative emergency stenting of the vertical vein in obstructive supracardiac pulmonary venous drainage prior to surgical repair

Abstract: Obstructed “total anomalous pulmonary venous connection” mostly presents with respiratory distress secondary to pulmonary congestion. We report two very sick patients who were referred to our cardiac center for intervention to relieve vertical vein (VV) obstruction. Due to hemodynamic instability and associated morbidity, the decision in our combined meeting was to go for interventional cardiac catheterization.

“…To our knowledge this is the first report of stenting of both VV and DV. This approach should not be considered only for LBW and premature neonates, 4 but this may present a bridge to a safer repair in all obstructed TAPVC to allow repair under less critical circumstances when the initial pathophysiologic process is reversed, which may improve outcomes in these challenging population 5 …”

“…Infants with obstructive total anomalous pulmonary venous connection are the sickest and carry significantly high mortality after surgery. Some of these infants may benefit from transcatheter relief of obstruction before surgery especially very sick, small premature infants with co-morbidities or with isomeric hearts [6][7][8][9][10][11]. The idea of preoperative stabilization by temporarily relieving the obstruction was developed to improve outcomes for neonatal obstructed total anomalous pulmonary venous connection [12].…”

“…2,3 Relief of pulmonary venous obstruction via emergent vertical vein stenting to stabilise a critical neonate prior to surgery can decrease morbidity and mortality. 4,5 We report the successful relief of severe pulmonary venous obstruction in a neonate with infradiaphragmatic total anomalous pulmonary venous connection, whom we felt was unfit for emergent surgery.…”
