In neonatology, neonates have traditionally been considered incapable of feeling pain, due to the immaturity of their nervous system. Currently, there is sufficient information on the perception of pain in neonates; however, this treatment at this crucial stage for development requires a better approach. For this reason, the aim of this study was to analyse the efficacy of non-pharmacological analgesia interventions during heel prick, and to assess their effects on heart rate (HR), premature infant pain profile (PIPP) and O2 saturation. A systematic review and meta-analysis was performed following the guidelines of the preferred reporting items for systematic reviews and meta-analyses (PRISMA), and the Cochrane collaboration handbook. The databases PubMed, Cochrane Library, Web of Science, Scopus, CINAHL and Science Direct were searched until the end of January 2022. The DerSimonian and Laird methods were used to estimate the effect size with a 95% confidence interval (CI95%). Effect size estimates were 0.05 (95% CI: −0.19, 0.29) for HR, −0.02 (95% CI: −0.24, 0.21) for PIPP scale, and −0.12 (95% CI: −0.29, 0.05) for O2 saturation. The non-pharmacological interventions analysed (breastfeeding, kangaroo-mother care method, oral sucrose and non-nutritive sucking) were not statistically significant in reducing neonatal pain, but did influence the decrease in pain score and a faster stabilisation of vital signs.

Introduction: In the Mother-Newborn Intensive Care Unit(M–NICU), sick, low birth weight(LBW) neonates are cared for along with their mothers with facilities of level-II newborn care and postnatal care for mothers. Babies weighing 1–1.8 kg are admitted and Kangaroo Mother Care (KMC) is started after birth without waiting for stabilization. This is immediate Kangaroo Mother Care (iKMC). There are studies on analgesic effect of KMC, but no studies on analgesic effect of iKMC in sick neonates; therefore, the present study was planned. Aims and objectives: To compare pain score and recovery time from pain following heel prick in sick LBW neonates weighing 1–1.8 kg in iKMC and under radiant warmer. Materials and methods: In the M-NICU, neonates are in iKMC for 12–18 hours. If iKMC is not possible, the baby is placed under radiant-warmer. LBW neonates are screened for hypoglycemia at 2, 6, 12, 24, 48-hour intervals by heel prick. Pain scoring was done by the Premature Infant Pain Profile (PIPP) score and the recovery time (time for heart rate to return to baseline) noted following heel prick in iKMC and under radiant warmer. Results: 75 babies enrolled, 150 episodes of glucose estimation in iKMC and 150 in radiant warmer included. Mean(SD) baby weight(Kg) was 1512.93(171.10). Mean(SD) gestational age(Weeks) was 33.77(2.54). In the iKMC group, mean pain score(SD) was 7.59(2.27) and median(IQR) was 8.00(2.75). In the radiant warmer group, mean pain score(SD) was 14.41(2.04) and median(IQR) was 15.00(3.00). Mean recovery time(seconds)(SD) was 33.3(20.22) in the iKMC group and 92.43(49.68) in the radiant warmer group. The difference was statistically significant( P = <0.001). Conclusion: In sick LBW neonates, painful procedures must be done in KMC, whenever possible.

Objective To study the effects of vitamin D supplementation on physical growth and neurologic development of very preterm infants receiving nesting intervention in the neonatal intensive care unit (NICU). Methods A total of 196 preterm infants had been hospitalized in NICU with the gestational age (GA) between 28 and 32 weeks. Among them, 98 preterm infants received nesting intervention, and the other 98 cases received both nesting and vitamin D supplementation (400 IU). The interventions were continued until 36 weeks postmenstrual age (PMA). The 25(OH)D serum levels, anthropometric parameters, and Premie-Neuro (PN) scores were compared at 36 weeks PMA. Results Higher median serum level of 25(OH)D was found in the nesting + vitamin D [38.40 ng/mL (IQR: 17.20 ~ 70.88) ng/mL] as compared to the nesting group [15.95 ng/mL (IQR: 10.80 ~ 24.30) ng/mL] at 36 weeks PMA. Besides, infants receiving combined nesting intervention and vitamin D supplementation had less proportion of vitamin D deficiency [VDD, 25(OH)D levels < 20 ng/mL] than those receiving nesting intervention alone. After intervention, the anthropometric parameters of infants, including weight, length, BMI and head circumference were improved in the nesting + vitamin D group as compared to the nesting group at 36 weeks PMA, with higher scores of neurological, movement and responsiveness. Conclusions Vitamin D supplementation effectively decreased the prevalence of VDD and led to higher concentrations of 25(OH)D at 36 weeks PMA. This was one more study that supported the necessity of vitamin D supplementation to improve physical growth and neurologic development of preterm-born newborns who received nesting intervention in the NICU.
