Efficacy of ultrasound-guided TAP block for postoperative pain relief in abdominal surgeries: A prospective, randomized controlled trial

: Transversus abdominis plane (TAP) block has emerged as a safe, reliable, and efficient technique to provide post-operative analgesia for a range of abdominal procedures and has been shown to minimize the usage of opioids in the perioperative period. This paper compares the overall efficacy and safety of TAP block for postoperative analgesia in abdominal surgeries, by two techniques (blind v/s USG). Eighty patients, ASA grade I-II, 18-60 years age group, posted for abdominal surgery like appendicectomy, appendicular perforation, umbilical, paraumbilical, incisional and ventral hernia repair, hysterectomy and exploratory laparotomy under GA. They were divided into two groups to undergo blind or USG-guided TAP block. At the end of the procedure, before the reversal, both groups received a TAP block with Inj. Bupivacaine 0.25% 20cc on each side in supine position. Patients were followed up for 24 hours, and pain scores were measured using a visual analogue scale. Inj. Diclofenac was given as rescue analgesic and Inj. Tramadol was used for breakthrough pain. Total analgesic requirement for 24 hours and complications if any, were noted.: VAS score was found to be significantly lower in USG- guided group at various time intervals till 12 hours (2.05 ± 0.75 vs 2.98 ± 1.03) in the USG-guided group as compared to the blind group (p<0.05). Time to first rescue analgesic was significantly prolonged in USG- the guided group being 19.68 ± 4.90 hours than the blind technique of 13.48 ± 6.86 hours (p <0.001). The number of rescue analgesics required in the USG-guided group was significantly lower than the blind technique (p<0.05).: USG-guided group had significantly less pain scores postoperatively and a reduced number of analgesic requirements. This resulted in fewer opioid-mediated side effects. TAP block can serve as a part of multimodal analgesia with enhanced recovery after abdominal surgery. The USG-guided approach helped in achieving near perfect block which is evident by pain scores and reduced analgesics required.

Dental Hydrogels with Essential Oils with Potential Activity in Periodontitis

This study seeks to understand the effects of new hydrogels based on Oregano® essential oil, Frankincense® essential oil, and the Thieves® blend, which includes clove, lemon, cinnamon bark, eucalyptus radiata, rosemary extract, and Frankincense® essential oil through in vitro cytotoxicity and antimicrobial analysis. Mesenchymal stem cells (D1MSCs) generated from the dental papilla of human wisdom teeth germs were used to test the hydrogels’ cytotoxicity (D1MSCs). The chemical makeup of the tested essential oils (EO) was determined using GC-MS analysis, and their presence in the novel hydrogels was determined using UV-VIS analysis. The primary constituents of the essential oils identified as being present were eugenol, pinene, limonene, carvacrol, and cymene. The primary constituents from essential oils identified by the absorption spectra included eugenol, pinene, limonene, carvacrol, and cymene. The presence of essential oils in the hydrogel composition was also clearly discernible. All of the studied compounds had a reduced cytotoxic effect on cell cultures, proving the lack of in vitro toxicity of the gels. This study demonstrates that hydrogels enriched with pure natural extracts of essential oils have a tangible in vitro antimicrobial potential, especially for Staphylococcus aureus and Bacillus cereus.

Improvement of the Nutraceutical Profile of Brewer’s Spent Grain after Treatment with Trametes versicolor

Brewer’s spent grain (BSG) is an important secondary raw material that provides a readily available natural source of nutraceuticals. It finds its largest application as animal feed and part of the human diet, while the future perspective predicts an application in the production of value-added products. In order to investigate a sustainable BSG treatment method, two BSG samples (BSG1 and BSG2) were evaluated as substrates for the production of hydrolytic (xylanase, β-glucosidase and cellulase) and lignolytic enzymes (laccase, manganese peroxidase and lignin peroxidase) by solid-state fermentation (SSF) with Trametes versicolor while improving BSG nutritional value. The biological treatment was successful for the production of all hydrolytic enzymes and laccase and manganese peroxidase, while it was unsuccessful for the production of lignin peroxidase. Because the two BSGs were chemically different, the Trametes versicolor enzymes were synthesized at different fermentation times and had different activities. Consequently, the chemical composition of the two BSG samples at the end of fermentation was also different. The biological treatment had a positive effect on the increase in protein content, ash content, polyphenolic compounds, and sugars in BSG1. In BSG2, there was a decrease in the content of reducing sugars. Cellulose, hemicellulose, and lignin were degraded in BSG1, whereas only cellulose was degraded in BSG2, and the content of hemicellulose and lignin increased. The fat content decreased in both samples. The safety-related correctness analysis showed that the biologically treated sample did not contain any harmful components and was therefore safe for use in nutritionally enriched animal feed.
