P.R.L. Platelet Rich Lipotransfert: Our Experience and Current State of Art in the Combined Use of Fat and PRP

Abstract: The authors report their experience about the use of P.R.L. PLATELET RICH LIPOTRANSFERT method (platelet rich plasma mixed fat grafting) in 223 patients affected by soft tissue defects (ulcers, Romberg syndrome, Hemifacial atrophy, loss of substance, and signs of aging). This paper introduces the reader to PRP therapy and reviews the current literature on this emerging treatment modality, showing at the current clinical use of PRP in plastic and reconstructive surgery, with description of innovative methods a… Show more

“…7,8 In the absence of standardized procedures, multiple surgical procedures and techniques are used in fat grafting, considering that adipose tissue can be purified by gravity or by different means of centrifugation. [11][12][13] Other approaches are based exclusively on the injection of adipose-derived mesenchymal stem cells after in vitro expansion. To date, it is widely known that some surgeons are grafting only the stromal vascular fraction of the adipose tissue, without the mature adipocytic component.…”

“…MSCs are attractive therapeutic agents, not only because of their potential for self-renewal and differentiation activities, but also for MSCs providing the proper microenvironment "niche" to enhance regeneration of cells or tissues damaged or cancer therapy through various signaling pathways and cellular events [31][32][33]. Therefore, diverse researches were tried to determine appropriate MSCs derived from various tissue by enhancing self-renewal, differentiation and anti-in ammation ability of MSCs, which is directly correlated with tissue regeneration [34,35]. Recently, placenta-derived MSCs, which are a representative source of fetal stem cells, became regarded as an alternative to bone marrow-derived MSCs in potential medical applications.…”

“…PLTs, primarily known for their contribution to hemostatis, are also able to release factors that promote tissue repair and regeneration and angiogenesis [5,41]. PRP acts as a trigger for the early development of a new capillary plexus, facilitating oxygen and nutrient diffusion towards the grafted cells [87,88,90].…”
