Oxygenation Status of Cervical Carcinomas Before and During Spinal Anesthesia for Application of Brachytherapy

Abstract: This study shows for the first time that the oxygenation status of cervical carcinomas, in general, is not influenced by spinal anesthesia prior to application of brachytherapy. To conclude, the data presented suggest that reliable pO2 measurements can be performed under spinal anesthesia. At the same time, since no substantial changes in tumor oxygenation were observed, spinal anesthesia should not affect the O2-related efficacy of high-dose-rate brachytherapy.

“…The potential to improve local control and survival by hypoxia modification was demonstrated by a meta-analysis of 83 clinical trials [23]. Tumor hypoxia is determined in a number of clinical and experimental settings [1,17,22,40] and correction for hypoxia is an important issue in the clinic [2,7]. A number of therapeutic strategies have been established to determine and to overcome tumor hypoxia by improving oxygen supply either by oxygen or carbogen breathing or by increasing the hemoglobin level and oxygen delivery [6,16,30].…”

“…In R1H recurrences oxygenation was determined, when they reached the same size as the previously treated primary tumors. To minimize the effects of anesthesia, the pO 2 measurements were carried out under controlled mechanical ventilation and continuous monitoring of hemodynamic parameters [22,31]. After a preceding anesthesia with ketamine and xylazine (see above), anesthesia was continued by an infusion of fentanyl die Primärtumoren erreichten (V o = 3,1 ± 0,5 cm 3 ).…”
