BackgroundWhat is common to many healthcare systems is a discussion about the optimal balance between public and private provision. This paper provides a scoping review of research comparing the performance of public and private hospitals in Europe. The purpose is to summarize and compare research findings and to generate questions for further studies.MethodsThe review was based on a methodological approach inspired by the British EPPI-Centre’s methodology. This review was broader than review methodologies used by Cochrane and Campbell and included a wider range of methodological designs. The literature search was performed using PubMed, EconLit and Web of Science databases. The search was limited to papers published from 2006 to 2016. The initial searches resulted in 480 studies. The final sample was 24 papers. Of those, 17 discussed economic effects, and seven studies addressed quality.ResultsOur review of the 17 studies representing more than 5500 hospitals across Europe showed that public hospitals are most frequently reported as having the best economic performance compared to private not-for-profit (PNFP) and private for-profit (PFP) hospitals. PNFP hospitals are second, while PFP hospitals are least frequently reported as superior. However, a sizeable number of studies did not find significant differences. In terms of quality, the results are mixed, and it is not possible to draw clear conclusions about the superiority of an ownership type. A few studies analyzed patient selection. They indicated that public hospitals tend to treat patients who are slightly older and have lower socioeconomic status, riskier lifestyles and higher levels of co-morbidity and complications than patients treated in private hospitals.ConclusionsThe paper points to shortcomings in the available studies and argues that future studies are needed to investigate the relationship between contextual circumstances and performance. A big weakness in many studies addressing economic effects is the failure to control for quality and other operational dimensions, which may have influenced the results. This weakness should also be addressed in future comparative studies.

BackgroundThe complexity of providing medical care in a high-tech environment with a highly specialized, limited labour force makes hospitals more crisis-prone than other industries. An effective defence against crises is only possible if the organizational resilience and the capacity to handle crises become part of the hospitals’ organizational culture. To become more resilient to crises, a raised awareness—especially in the area of human resource (HR)—is necessary. The aim of this paper is to contribute to the process robustness against crises through the identification and evaluation of relevant HR crises and their causations in hospitals.MethodsQualitative and quantitative methods were combined to identify and evaluate crises in hospitals in the HR sector. A structured workshop with experts was conducted to identify HR crises and their descriptions, as well as causes and consequences for patients and hospitals. To evaluate the findings, an online survey was carried out to rate the occurrence (past, future) and dangerousness of each crisis.ResultsSix HR crises were identified in this study: staff shortages, acute loss of personnel following a pandemic, damage to reputation, insufficient communication during restructuring, bullying, and misuse of drugs. The highest occurrence probability in the future was seen in staff shortages, followed by acute loss of personnel following a pandemic. Staff shortages, damage to reputation, and acute loss of personnel following a pandemic were seen as the most dangerous crises.ConclusionsThe study concludes that coping with HR crises in hospitals is existential for hospitals and requires increased awareness. The six HR crises identified occurred regularly in German hospitals in the past, and their occurrence probability for the future was rated as high.

ResumenObjeto: Analizar y estudiar si las políticas sociales de bienestar repercuten en la eficacia y eficiencia de las empresas que prestan servicios a las personas con dependencia (Código CNAE 87 y 88).Diseño/metodología/enfoque: Para la realización del estudio se han comparado las empresas de los sectores que dan servicios en países que presentan pautas diferentes a la hora de aplicar determinadas políticas sociales. En concreto se han comparado las empresas que actúan en España, Alemania, Inglaterra y los Países Bajos. Los datos se han obtenido a partir de la base de datos AMADEUS. Se ha tomado como período de referencia el 2008-2012. Las variables utilizadas han sido la tasa de margen operativo (TMO) y la tasa de rotación de activos (TRA). Aportaciones y resultados:De los resultados obtenidos no se ha podido establecer una relación estadísticamente significativa entre la eficacia y eficiencia de las empresas que prestan servicios a personas con dependencia y las políticas sociales desarrolladas desde los gobiernos.Originalidad / Valor añadido: : Hasta el presente existen pocos estudios sobre el comportamiento financiero de las empresas que prestan servicios a la tercera edad. El desarrollo empresarial de estas actividades implica un crecimiento de la actividad económica que compensa los costes que comporta el crecimiento de la población.
