Over-triage occurs when considering the patient's pain in Korean Triage and Acuity Scale (KTAS)

Abstract: Background The Korean Triage and Acuity Scale (KTAS) was developed based on the Canadian Emergency Department Triage and Acuity Scale. In patients with pain, to determine the KTAS level, the pain scale is considered; however, since the degree of pain is subjective, this may affect the accuracy of KTAS. The purpose of this study was to evaluate the accuracy of KTAS in predicting patient's severity with the degree of pain used as a modifier. Method A retrospective observa… Show more

“…ureter stone with severe pain). Therefore, predictability of KTAS in a previous study may be the result of overtriage when compared with outcomes included emergency operation, the admission rate, the 7-day mortality [20]. In our study, the frequency of mistriage related to the pain scale was more related to under-triaged than to over-triage.…”

“…The major cause of mistriage in this study was the wrong application of the pain scale. In one study, a degree of pain on KTAS resulted in overtriage and had a negative influence on the predictability [20]. Patents who had uncomplicated diseases with severe pain were considered urgent but often did not have negative outcomes.…”

“…According to research by Lee et al [25], overestimation of the level of triage may occur since KTAS was based on the subjective evaluation of pain. In this study, using an ambulance is determined according to a patient's subjective judgment, and pain was already considered as a source of bias in the selection of the transportation mode for ED visits.…”

“…Five-level triage systems, including the Canadian Triage and Acuity Scale (CTAS), Manchester Triage System (MTS), and emergency severity index (ESI), are widely used [2,8,9]. The Korean Triage and Acuity Scale (KTAS) was developed in 2012 based on CTAS and has been used nationally as the ED triage tool in Korea since 2016 [11][12][13]. Although five-level triage systems are well established in ED, they need to be improved because they heavily rely on healthcare providers' subjective judgment, resulting in high variability [5,[7][8][9][10]12].…”
