Outcomes, Union Rate, and Complications After Operative and Nonoperative Treatments of Neer Type II Distal Clavicle Fractures: A Systematic Review and Meta-analysis of 2284 Patients

Abstract: Background: As nonoperative treatment of Neer type II distal clavicle fractures is associated with nonunion rates up to 33%, operative treatment is frequently advocated. However, evidence is lacking regarding which operative treatment to perform and whether this is superior to nonoperative treatment in terms of functional outcome and complication rate. Purpose: (1) To evaluate which surgical technique in the treatment of Neer type II distal clavicle fractures is optimal with regard to patient-reported outcomes… Show more

“…A recent systematic review by Uittenbogaard et al (2021) [ 19 ] in 2284 patients, including all Neer type II fractures (IIA and IIB), showed lower CS of the hook plate in comparison to CC fixation and also better clinical results when a standard locking plate was augmented with CC fixation; the latter conjecture was not confirmed in the most recent systematic review and meta-analysis (2022) conducted by Panagopoulos et al [ 12 ] in our department. Specifically, this systematic review on the best available treatment options for Neer type IIB/IIC distal clavicle fractures showed not only a low level of evidence and heterogeneity among studies but also inconsistent methods of classification, inappropriate outcome evaluation, and underreporting of complications.…”

“…4,24,27 Moreover, tension band wire fixation constructs are associated with significant complication rates of 62%. 25 An alternative option to achieve fixation of lateral end clavicle fracture is CC fixation. Bosworth 3 initially described CC fixation with screws, which subsequently were reported to attain a 100% union rate.…”

“…4,24,27 Tension band wire fixation is associated with notably higher complication rates, with a reported hardware failure rate of 62% and wound complication rate of 9%. 25 The ideal implant for lateral clavicle fractures should provide enough stability for fracture union with minimal complication rates and elective plate removals. CC fixation can be achieved with several constructs such as screws or sutures.…”

“…Distal clavicle fractures are among the most common injuries in adults, accounting for 2.6–5% of all fractures and 21% of clavicle fractures [ 1 5 ]. It constitutes up to 45% of clavicle non-unions and can result in severe disability if not treated adequately; therefore, unstable distal clavicle fractures should receive appropriate surgical treatment [ 2 , 6 , 7 ]. Distal clavicle is anatomically made of metaphyseal bone with a small distal fragment.…”
