The aim of this chapter is to define hemodiafiltration target efficiency, to clarify the concept of "optimal convective dose," and to facilitate hemodiafiltration (HDF) implementation in clinical practice by addressing the need for the establishment of best clinical practices for HDF. The approach taken was to conduct a comprehensive summary of clinical evidence supporting HDF. Convective dose is the total ultrafiltered volume and is complementary to diffusive dose (urea Kt/V) as a dose-dependent parameter. It can be quantified and adjusted to patient characteristics. Factors affecting convective dose are discussed: patient characteristics, prescription-dependent factors, and technical and machine-dependent factors. The key issue of HDF prescription and implementation of best practices is addressed as are intermediary and endpoint clinical outcomes. The main messages are as follows: (1) HDF is safe and effective provided that best clinical practices are followed and the right convective dose is delivered; (2) HDF is easy to perform with new technology; and (3) depending on the convection volume, HDF reduces all-cause and cardiovascular mortality. Open challenges remain, namely, the implementation of best practices to (a) achieve optimal convection volume, (b) define patient subsets that would benefit more from HDF, and (c) evaluate new tools that fine-tune HDF prescription according to individual patient needs.

Безопасность ОЛ-ГДФ целиком основывает-ся на строгом и постоянном соблюдении правил проведения процедуры и обслуживания оборудо-вания. В настоящее время около 15% диализных пациентов Западной Европы получают ОЛ-ГДФ, в России эту методику практикует треть центров ге-модиализа (ГД). Ограничивающим фактором для более широкого применения этого метода явля-ются законодательно-административные барьеры на использование приготавливаемой онлайн заме-щающей жидкости [1]. Главный принцип онлайн-приготовления замещающего раствора -постоян-ная стерилизация с помощью ультрафильтрации (УФ) и непосредственное использование -не рассматривается Фармакопеей в качестве стери-лизационного метода. Изготовление лекарства (замещающий раствор) с помощью аппарата (из диализирующей жидкости) с использованием дру-гого устройства (ультрафильтр) -процесс, выхо- Вторая часть обзора посвящена правовым и финансовым аспектам онлайн-гемодиафильтрации, а также ее влиянию на непосредственные и отдаленные результаты лечения. Недавно завершенные рандомизи-рованные исследования подтвердили, что высокообъемная онлайн-гемодиафильтрация позволяет улуч-шить выживаемость пациентов. Эти данные должны переломить инерцию, препятствующую повсемес-тному внедрению онлайн-гемодиафильтрации в качестве основной методики в практику программного гемодиализа.Ключевые слова: онлайн-гемодиафильтрация, регулирование, клиническая эффективность. Second part of review summarizes legal and fi nancial aspects of online hemodiafi ltration and its infl uence on short-term and long-term treatment results. According to recently completed randomized clinical trials, high volume online hemodiafi ltration is able to improve patients' survival rate. The results mentioned in the review must overpower the inertia which obstructs a wide implementation of online hemodiafi ltration into routine clinical practice. Hemodiafiltration (HDF) was created in the late 1970s to combine the best of hemodialysis (HD) and hemofiltration, i.e. superior removal of both small and large solutes. Thus, optimal conditions for both diffusion and convection are required. The need for large volumes of sterile fluid has shaped the evolution of HDF, and regulatory issues around systems for on-line fluid preparation have delayed the clinical development of the therapy. Not until such systems became generally available in the mid-1990s was it possible to increase the efficiency of HDF therapy to match and exceed the upgrade that had taken place for HD. Results from recent studies indicate that the potential for improving the outcome of dialysis lies in increasing the convective clearance of HDF above that of high-flux HD.
