Outcomes following laparoscopic transhiatal esophagectomy for esophageal cancer

Abstract: LTE can be performed safely with less major complications and shorter hospital stay than open esophagectomy. The reduced lymph-node harvest did not impact overall survival.

“…There have been four comparative studies of conventional open and laparoscopic THE. (17)(18)(19)(20)(21) Based on these studies, the laparoscopic THE group had significantly less blood loss than the open THE group, but no differences in operation times were evident between the groups. Although overall morbidity tended to be lower in the laparoscopic THE group, the pulmonary complication rates did not differ.…”

“…Since the first report by DePaula et al in 1995, many studies on laparoscopic THE have been reported, mostly for distal and junction cancer in Western countries. There have been four comparative studies of conventional open and laparoscopic THE . Based on these studies, the laparoscopic THE group had significantly less blood loss than the open THE group, but no differences in operation times were evident between the groups.…”

“…Furthermore, by avoiding patient's repositioning during the procedure, this approach benefits from a shorter operative time [12 • ]. However, this comes at the cost of a poorer visualization of the intrathoracic esophagus and minimal lymphatic dissection in the thorax [13]. Data from one of the largest studies comparing laparoscopic with open transhiatal esophagectomies showed that transhiatal MIE was associated with a shorter hospital and intensive care unit stay, with similar operative time and morbidity [14].…”

“…Also of note, the magnified visualization offered by the laparoscopic camera may allow for less 'blind' mediastinal dissection, therefore reducing blood loss. This is particularly relevant in transhiatal MIE compared to other MIE techniques that involve direct thoracic visualization [13].…”
