Outcomes After Intravesical Bacillus Calmette-Guerin Are Not Affected by Substaging of High Grade T1 Transitional Cell Carcinoma

Abstract: Substaging of T1 tumors did not affect response to BCG in regard to recurrence or progression. Therefore, intravesical BCG is effective for stages T1a and T1b disease.

“…The depth of lamina propria invasion has been significant in multivariable analyses for overall survival in three studies using the substaging system based on MM identification [9,16,21]. Recurrence rates and recurrence-free intervals did not differ significantly for either group in our study, which was also in agreement with the results published previously [7,8,18,24,25,28]. …”

“…A second proposal divides T1 patients into two populations only. Tumours superficial to the MM or reaching the MM but not passing through it were classified as pT1a, and tumours passing through the MM and invading the submucosa were considered to be pT1b [16,21,24]. A third proposal subclassifies T1 tumours as either with MM invasion (pT1b) or without (pT1a) [6,7,17,20,25,26].…”

“…Similarly, Kondylis et al . found no significant difference in recurrence and progression in their study, with a median follow‐up of 71 months . Conversely, in the largest of these studies, involving 587 pT1 patients from six hospitals and median follow‐up of 35 months, Rouprêt et al .…”

“…Furthermore, the location of the vascular plexus can vary greatly either above or below the muscularis mucosae, particularly in the trigone, making the use of the vascular plexus questionable . Nonetheless, beginning in 1990 with Younes et al ., many studies have investigated pT1 substaging relative to the muscularis mucosae and/or vascular plexus with mixed, but generally positive, results …”
