Outcome of graft failure after allogeneic stem cell transplant: study of 89 patients

Abstract: Strategies for reversing graft failure (GF) after allogeneic stem cell transplant (SCT) depend on the options available in each situation. GF was reported in 16 Spanish institutions from January 2006 to July 2011. Primary GF was defined as an absolute neutrophil count (ANC) > 0.5 × 10(9)/L not reached by day + 28 after SCT from peripheral blood (PB) or bone marrow (BM) progenitors and by day + 42 after SCT from unrelated cord blood (UCB) progenitors. Secondary GF was defined as a recurrent ANC < 0.5 × 10(9)/L.… Show more

“…Second allo-SCT or infusions of unmanipulated peripheral blood stem cells are other options but are associated with a high risk of GvHD and non relapse mortality 6 . Larocca et al 7 reported the use of CD34 + -selected stem cell infusions from the original donor without conditioning for correction of poor GF based upon the premise that the risk of GvHD would be low.…”

“…Cord blood, unrelated donors, or haploidentical transplantation are alternatives. However, the literature is scarce on the subject, and the long-term overall survival (OS) of retransplanted patients is estimated to be about 30% [4].…”

“…[1][2][3] As a salvage therapy, second allogeneic transplantation (alloSCT2) results in high rates of non-relapse mortality (NRM). [1][2][3] As a salvage therapy, second allogeneic transplantation (alloSCT2) results in high rates of non-relapse mortality (NRM).…”

“…Graft failure is a rare but potentially life-threatening complication of allogeneic stem cell transplantation (alloSCT) occurring in approximately 5% of transplants. [1][2][3] As a salvage therapy, second allogeneic transplantation (alloSCT2) results in high rates of non-relapse mortality (NRM). [3][4][5][6][7][8] Haploidentical transplantation utilising posttransplant cyclophosphamide (PTCy) appears to result in low NRM in the upfront setting and may similarly reduce NRM in the salvage alloSCT context.…”
