Out-of-Hospital Cardiac Arrest From Brain Cause

Abstract: Presentation of cerebrovascular event complicated with out-of-hospital cardiac arrest may mimic coronary etiology, but several clinical elements may help to identify brain causes. Even if survival is null, the high proportion of brain deaths provides opportunity for organ donation.

“…As the location of reported death was related to brain in 89%, unspecific intracranial hemorrhage caused death in 41% and traumatic intracranial hemorrhage caused death in 27%. Arnaout et al, in 2015 confirmed that presentation of cerebrovascular event may help to identify brain causes [13]. Christe et al, in 2010 mentioned that intracranial bleeding is hyperattenuating both in radiology and in postmortem imaging [14].…”

“…However, although cardiac causes of OHCA have been extensively studied in recent years, little is known regarding non-cardiac causes; therefore, the current 2015 guidelines point out that early identification of respiratory and neurological causes might be achieved by performing early computed tomography (CT) [5]. At present, only a few reported case series have demonstrated that early CT examination for victims of OHCA before or after coronary angiography enables the diagnosis of non-cardiac causes of arrest in a substantial proportion of patients [6,7]. The overall diagnostic value of early CT in victims of non-traumatic OHCA remains uncertain, and early CT may provide more therapy-relevant findings than only the cause of cardiac arrest.…”

The previous largest study (86 patients) was conducted to identify the unique features of OHCA of neurological causes to determine the optimal care provisions.…”
