This review underlines the significant role of early diagnosis and treatment of hearing impairment due to bacterial meningitis (BM) in children. BM is a life-threatening neurological condition mostly caused by Streptococcus pneumoniae, Neisseria meningitides, or Haemophilus influenzae. Hearing loss is the most commonly reported neurological complication of BM; inflammation can damage the inner ear, leading to sensorineural hearing loss or complete deafness. Factors favoring neurological complications, including hearing impairment, are low age, immaturity of the immune system, poor health, anaemia, leukocytosis, and hypoglycemia. To avoid serious complications, quick intervention is necessary – administration of antibiotic in combination with dexamethasone. It is also important to conduct regular audiological tests to monitor hearing, not only immediately after BM, but also in the long term. Otoacoustic emissions (OAEs) and auditory brainstem responses (ABRs) are useful tools, and some researchers also recommend tympanometry. CTs and MRIs are important to visualize the condition of the inner ear after BM, paving the way for qualification for implantation and for pre-operative planning. The best results of implantation come from patients without ossification, whose period of deafness was short, where electrodes were inserted deeply, and who did not have neurological complications after BM. Early implantation is the best option as it promotes proper speech development and allows the child to adapt to their environment.

Background Hearing loss and deafness are well-known sequelae from bacterial meningitis (ABM) and may result in social dysfunction and learning difficulties. Yet, the timely development of hearing loss and restitution is poorly studied, especially among adults. Hearing loss was revisited using otoacoustic emissions (OAE) to determine the occurrence, magnitude, and development of hearing loss among adults with ABM. Methods Distortion product otoacoustic emissions (OAE) were measured in patients with ABM the day of admission and days 2, 3, 5-7, 10-14 and at follow up 30 to 60 days after discharge. Frequencies were categorized as Low (1,1.5,2 kHz), Mid (3,4,5 kHz), Mid-high (6,7,8, kHz) and High (9,10 kHz). Audiometry was performed on discharge and 60 days after. Results were compared to 158 healthy controls. Results OAE was obtained in 32 patients. ABM was due to S. pneumoniae in 12 patients (38%). All patients were treated with dexamethasone. OAE emission threshold levels (ETL’s) were significantly decreased upon admission and at follow up in all frequencies compared to healthy controls. A substantial and significant decrease in ETL’s was found in S. pneumoniae meningitis. Sensorineural hearing loss (SNHL) >20 dB was present in 13 of 23 (57%) at discharge and in 11 of 18 patients (61%) 60 days after discharge. Hearing recovery decreased from day 3. Conclusion Hearing loss in ABM still affects more than 60% of patients despite treatment with dexamethasone. In S. pneumoniae meningitis SNHL is profound and permanent. A window of opportunity for systemic or local treatments aiming to preserve cochlear function is proposed.

We describe the case of a patient who presented with sudden onset vertigo and bilateral deafness while under immune checkpoint therapy for metastatic bronchial carcinoma. Extensive audiologic assessment and vestibular function testing, as well as cranial magnetic resonance imaging (cMRI) and lumbar puncture was performed. The diagnostic workup confirmed bilateral sensorineural deafness and bilateral loss of vestibular function, while imaging revealed enhancement of the vestibulocochlear nerve. Initially, immunotherapy with PD‐L1 antibody atezolizumab was the assumed cause of the described symptoms. However, further findings strengthened the suspicion of meningeosis neoplastica. The differential diagnoses of hearing loss and vestibulopathy in the context of platin‐based chemotherapy, checkpoint inhibitor therapy and metastatic disease should be kept in mind for appropriate workup and therapy.
