Epilepsy is a chronic condition that consists of a transient disorder of brain function manifested by recurrent and unprovoked seizures affecting approximately 50 million people worldwide. Antiepileptic treatment is aimed at possible improvement the quality of life by reducing epilepsy seizures with the lowest possible side effects. One of the newest broad spectrum antiepileptic drug (AED) for the treatment of wide range of epilepsies – focal, partial, myoclonic, tonic-clonic and primary generalized seizures that can be used in both children and adults is levetiracetam. Levetiracetam is believed to bind to SV2A (synaptic vesicle protein 2A) in the brain, prevent the release of calcium stores from neurons and inhibit GABA and glycine activity. Due to the unique mechanism of action, high bioavailability (in both oral and intravenous administration), rapid absorption, metabolism based on the hydrolysis of acetamide groups in the blood (independent of the liver) and the lack of activity of the resulting metabolites, the drug is considered to be safe and effective in reducing and stopping seizures. In this review, we present the latest reports on the effectiveness of levetiracetam and its potential in the treatment of epilepsy in children and adults.
Perlaki to wrodzone lub nabyte torbielowate, miejscowo złośliwe guzy nienowotworowe zlokalizowane najczęściej w obrębie ucha środkowego, wyrostka sutkowatego lub rzadziej w okolicy przewodu słuchowego zewnętrznego. U dzieci występują z częstością 3 na 100 000, a u dorosłych – 9,2 na 100 000 osób. Przyjmuje się, że perlak wrodzony dotyczy 2–4% ogólnej liczby perlaków i tworzy się przed urodzeniem za prawidłowo ukształtowaną błoną bębenkową. Mimo że pierwszy opisany przypadek perlaka datuje się na rok 1683, etiopatogeneza wciąż jest nie do końca poznana. Dotychczas powstało kilka teorii wyjaśniających powstanie perlaka wrodzonego – teoria migracji, inkluzji, tworzenia naskórka i skażenia płynu owodniowego, ekspresja czynników związanych z apoptozą lub ekspresja cząsteczek adhezyjnych naskórka. Obecnie do oceny zaawansowania choroby używa się klasyfikacji Levensona, Potsica lub Nelsona. Jedyną metodą leczenia perlaka jest leczenie chirurgiczne polegające na całkowitym usunięciu guza za pomocą endoskopowej lub mikroskopowej chirurgii ucha. Niezwykle istotne jest wczesne wykrycie zmiany, co umożliwia odpowiednio wczesne podjęcie mikroinwazyjnej operacji i pozwala w większości przypadków zachować prawidłowy słuch i uniknąć niebezpiecznych powikłań (tj. ciężkie uszkodzenie słuchu, zawroty głowy, niedowład twarzy, zapalenia opon mózgowo-rdzeniowych, powstawanie ropni i przetok etc.). Perlak wrodzony na wczesnym etapie rozwoju zwykle nie wykazuje żadnych objawów, stąd ważne wydawałoby się przeprowadzanie badań przesiewowych – otoskopii i badań obrazowych (CT/MRI). Wielu naukowców podkreśla rolę lekarza podstawowej opieki zdrowotnej (POZ) we wczesnym rozpoznaniu choroby za pomocą prostego badania otoskopowego u każdego pacjenta. W pracy przedstawiona zostanie etiopatogeneza, klasyfikacja i istota badań przesiewowych u dzieci dotkniętych perlakiem wrodzonym ucha środkowego.
Introduction. Schmidt›s syndrome is one of the APS-2 variants, occurring with the frequency of 1.5-4.5 / 100,000, diagnosed approximately twice as often in women. Family occurrence is observed. Schmidt›s syndrome is found in the form of Addison›s disease and autoimmune thyroid disease. The units that make up the syndrome can appear in any order and time, and overlapping symptoms make diagnosis difficult. Case Report. A 39-year-old patient with hypothyroidism and impending adrenal crisis was admitted to the Endocrinology Department for hormonal evaluation. In the history of about a year, weight loss of 20 kg, lack of appetite, weakness, hypotonia, slightly darker skin with a large number of freckles. Second amenorrhea for a year and symptoms of depression. Two weeks earlier, the patient was hospitalized in the neurology department due to confusion, hyponatremia and cerebral edema found on CT. Symptoms after administration of mannitol and hydrocortisone resolved. Moreover, the abdominal CT scan revealed hypotrophic adrenal glands. In the Department of Endocrinology, a low concentration of morning cortisol was confirmed with a high level of ACTH (1623 pg / ml), which allowed confirmation of the diagnosis of Addison's disease. In addition, Hashimoto's disease was found. The coexistence of both diseases allowed the diagnosis of Schmidt's syndrome. Conclusions. The probability that a patient with one autoimmune disease will be diagnosed with another entity from the autoimmune spectrum is very high. The presented patient had typical symptoms of adrenal insufficiency, but they were associated with malnutrition, hypothyroidism and depression. Often, the symptoms of Addison's disease are masked by hypothyroidism, which makes diagnosis difficult.
Wrodzona atrezja przewodu słuchowego zewnętrznego to wrodzona anomalia często współwystępująca z mikrocją występująca z częstością 1:10 000 do 1:20 000 urodzeń. Wada ta przyczynia się do niedosłuchu typu przewodzeniowego u dzieci, powoduje opóźnienie rozwoju mowy, utrudnienie relacji społecznych i pogorszenie wyników w nauce. Pomocne w diagnostyce atrezji jest badanie ABR, które pozwala na wczesne wykrycie wady i zaplanowanie postępowania terapeutycznego. W leczeniu atrezji przewodu słuchowego zewnętrznego odchodzi się od konwencjonalnej plastyki kanału z uwagi na wysoki odsetek pooperacyjnej restenozy w przypadku plastyki atrezji na rzecz innowacyjnych metod bez konieczności zachowania drożności kanału. Do alternatywnych sposobów leczenia wrodzonej atrezji przewodu słuchowego zewnętrznego zaliczyć można implanty słuchowe oparte na przewodnictwie kostnym ucha środkowego, wśród których wyróżnia się implanty zakotwiczone w kości i implanty przezskórne. Urządzenia te złożone są z wszczepionej części wewnętrznej i z części zewnętrznej (umieszczonej na skórze głowy). Mechanizm ich działania polega na odbiorze dźwięków ze środowiska przez zewnętrzną część urządzenia, które są następnie przekształcane w wibracje przekazywane przez kości czaszki do ucha wewnętrznego bez konieczności udziału ucha zewnętrznego i środkowego. Rozwój coraz nowszych technik operacyjnych pozwolił na zredukowanie powikłań po zabiegach wszczepiania implantów. W pracy przedstawiona zostanie patogeneza wrodzonej atrezji przewodu słuchowego zewnętrznego oraz najnowsze metody leczenia tego schorzenia z uwzględnieniem ich wad i zalet. RNA interference (RNAi) discovered in the 1990s by Fire and Mello plays a role in silencing gene function. One type of RNAi is siRNA, which is a double-stranded molecule of 20-25 base pairs. This molecule is made by cleaving double-stranded RNA by the enzyme Dicer. siRNA binds to the protein complex with ribonuclease activity - RISC (RNA-induced silencing complex). The resulting complex binds to the mRNA and cuts it into parts, which blocks the formation of the protein encoded by the mRNA. This property has been exploited in the production of siRNA-based drugs. However, the instability of siRNA molecules turned out to be difficult - the challenge was to properly modify the structure of siRNA in order to increase the stability and half-life and select the appropriate method of delivering the molecule to the body. Many siRNA-based drugs have already been developed, but most are in clinical trials. In this review, we present the role of RNA interference in therapeutics production and use of siRNA in cardiovascular diseases treatment.
