Osteomyelitis due to Shewanella putrefaciens: case report and literature review

Abstract: Shewanella putrefaciens is a Gram-negative bacillus and marine pathogen that rarely causes disease in humans. We report a case of osteomyelitis by this organism in a 48-year-old male patient, who presented with pain and erythema of the right foot that was initially diagnosed as cellulitis and did not revert despite treatment. He was transferred to Lima where osteomyelitis was diagnosed and started on empirical treatment with partial regression. A biopsy and culture of the compromised area found S. putrefaciens… Show more

“…Shewanella spp. can cause dangerous infections: one case of monomicrobial S. putrefaciens infective endocarditis and several cases of osteomyelitis have been diagnosed ( 6 , 7 , 11 ). P. shigelloides and P. rettgeri have been associated with gastroenteritis and diarrhoea in travellers who have ingested contaminated water or food ( 8 ).…”

“…Under certain circumstances, these can take fatal courses up to necrotizing fasciitis ( Yim et al., 2010 ; Giroux et al., 2017 ) and the development of a Fournier’s gangrene ( Tang et al., 2016 ). In addition, S. putrefaciens can cause arthritides and osteomyelitides ( Levy and Tessier, 1998 ; Carlson and Dux, 2013 ; Guinetti-Ortiz et al., 2016 ). Potential entry ports and a typical predisposing factors for corresponding infections are chronic ulcers, especially of the lower extremities, and/or traumatic injuries associated with (sea) water or fish (water) exposure ( Oh et al., 2008 ; Carlson and Dux, 2013 ; Vignier et al., 2013 ; Guinetti-Ortiz et al., 2016 ).…”

“…In severe courses, especially with existing sepsis, intravenous combination treatment using, for example, an aminoglycoside antibiotic should be considered ( Brink et al., 1995 ). Depending on resistance testing and the clinical course, oral sequential antibiotic therapy may be appropriate in special cases ( Guinetti-Ortiz et al, 2016 ; Patel et al., 2020 ).…”
