Os Odontoideum in Children

Abstract: Os odontoideum is a rare entity of the second cervical vertebra, characterized by a circumferentially corticated ossicle separated from the body of C2. The ossicle is a distinct entity from an odontoid fracture or a persistent ossiculum terminale. The diagnosis may be made incidentally on imaging obtained for the workup of neck pain or neurologic signs and symptoms. Diagnosis usually can be made with plain radiographs. MRI and CT can assess spinal cord integrity and C1-C2 instability. The etiology of os odonto… Show more

“…The etiology of the os odontoideum is controversial, with most studies supporting the theory that the ossification center at the tip of the dens of C2 did not fuse by 12 years of age. 6 Associated hypertrophy of the anterior arch of the atlas, hypoplasia of the posterior arch, and well-corticated margins are main findings to differentiate it from a traumatic origin. Further, if patients present with neck pain or even myelopathy and/or the ossicle is displaced on lateral flexion and extension radiographs, a traumatic origin is most likely.…”

“…This subject of Proatlas‐manifestation, the clinical phenomena potentially resulting from it and their treatment is part of modern medicine. Always new findings of both morphological (Tsuang et al, 2011; Spittank et al, 2016) and clinical investigation (Hedequist & Mo, 2020; Menezes & Dlouhy, 2020; Montrisaet & Petcharunpaisan, 2020; Sato et al, 2019) continue to enrich this field of research.…”

“…Os Odontoideum (OO), known as a rare anomaly of the cervical spine and can be confused with fracture, involves the second cervical vertebrae, which is characterized by the separation of a portion of the odontoid process (also known as dens) from the body of the axis. 1,2 It was first described by Giacomini in 1886, and whether the etiology of OO is congenital or traumatic has remained controversial. [3][4][5] Notably, OO leads to the instability of atlanto-axial joint, which will increase the risk of spinal cord and vertebral artery injury.…”
