El presente caso clínico se enfoca en el diagnóstico yel tratamiento de erupción forzada en un paciente contrauma dental. ObjetivoProponer una alternativa de tratamientomultidisciplinario el cual permita incrementarestructura dentaria a través de la extrusión forzada yposteriormente rehabilitarlo en función y estética. MétodosSe describe el caso de un paciente adulto de 78años de sexo masculino, sano y sin antecedentespatológicos, acude a la clínica de la Especialidad deOrtodoncia y Ortopedia Dentomaxilofacial, referidopor un especialista en Endodoncia, debido a fracturacorono-radicular de canino superior derecho, pilar depuente fijo de tres unidadesEn la exploración intraoral presenta fractura cervicalde la corona del canino superior derecho. Comoprimer paso, se realizó tratamiento endodónticoen la pieza dentaria y colocación de un aditamentointraconducto de soporte (endoposte vaciado), con elobjeto de mejorar la tracción ortodóntica.Este aditamento consistió en un poste colado conperforaciones. Se procede a colocar aparatología fijaen la arcada superior con técnica MBT (slot 0.022),del molar derecho hasta el canino izquierdo decementado indirecto y pasivo (con los slots de losbrackets alineados). Inmediatamente después, secolocó un arco rectangular 0.019 x 0.025 de acero inoxidable con un doblez de extrusión anivel del canino superior derecho. En elmismo doblez, se adaptó un loop tipohelix que funcionó como apoyo paracolocar la ligadura pasiva (lace back). ResultadosEl tratamiento realizado en estepaciente es satisfactorio, coadyuvandoen su estado de salud generalmejorando su autoestima. ConclusiónAquí se aprovecharon todas las ventajasque ofrece la extrusión ortodónticaforzada, incluso en un paciente adultomayor, logrando una tracción de cuatromilímetros, que se consiguió gracias alempleo de fuerzas extrusivas ligeras ycontroladas sobre el órgano dentarioafectado.Con la modalidad de tratamientodescrita se puede lograr unalargamiento de corona sin la necesidadde realizar una resección ósea, lo quepermite una correcta rehabilitaciónprotésica, devolviendo la función yestética al diente lesionado y brindandoun beneficio integral al paciente.

Introduction:Fracture of tooth structure at or below the gingival margin compromises rehabilitation and hampers esthetics and function.Management:Management of such cases by a post-core and crown restoration, or periodontal surgery or orthodontic extrusion alone may not always suffice in attaining a good result.Case Report:A multi-disciplinary approach which includes all of the above mentioned procedures helps in long term success.Conclusion:Careful case evaluation, treatment planning and meticulous attention to detail are the keys to the best treatment outcome.

Introduction: Esthetics has become an essential criterion of the overall treatment plan in dentistry that comprises of a healthy and beautiful smile at any age. Multidisciplinary approach involving orthodontics and periodontics results in the maintenance of a beautiful smile and a healthy periodontium. The continuous endeavor for innovation of newer interdisciplinary treatment moda lities has resulted in the use of a passive object like the orthodontic button being used to provide the initial stabilization in cases of root coverage using a Coronally advanced flap (CAF).Objective: The objective of this case report is to evaluate the effectiveness of a new treatment approach which consists of coronally advanced flap (CAF) procedure combined with ortho dontic button application for the treatment of Miller's class III recession defects.Methodology: After the application of orthodontic buttons in middle third of tooth surface, a split-full split flap was raised from tooth #22 to #24 and the flap was sutured 3 to 4 mm coronal to cementoenamel junction and the central part of flap was suspended with sutures to the orthodontic button to maximize the stabilization of the immediate postoperative flap location. Clinical parameters were recorded at 1 and 3 months postoperatively.Results: Complete root coverage was achieved when evaluated from baseline to 3 months along with gain in clinical attachment level and keratinized tissue. The final esthetics, both color match and tissue contours, were highly acceptable. Conclusion:Three months postoperative results showed that the CAF combined with orthodontic button is a very effective approach even in the treatment of Miller's Class III recession defects.
