Ce texte présente une partie des résultats d’une recherche menée auprès des Intervenants et intervenantes en soins spirituels du Québec (ISS) qui travaillent dans les établissements de santé et de services sociaux du Québec. Il présente l’analyse des récits de pratique et de journées typiques de 23 ISS aux profils et pratiques diversifiés. Les ISS sont les successeurs des aumôniers ou animateurs de pastorale hospitalière, aussi appelés chaplains, et ont pour particularité de devoir offrir des services de soins spirituels à tout usager de l’institution de santé, quelle que soit son appartenance ou non à une organisation religieuse. C’est ainsi qu’ils offrent un accompagnement « spirituel » et non « religieux », cette distinction étant fondamentale pour eux. Or, pour démontrer leur pertinence et leur « utilité », les ISS doivent actuellement faire montre de la particularité de leur travail d’accompagnement spirituel (par rapport à la psychologie ou au travail social, par exemple), mais aussi de leur capacité à le faire quelles que soient les croyances du patient. Pour ce faire, ils ont élaboré un savoir pratique, basé sur leur expérience auprès des patients et des soignants, et ont, pour ce faire, élaboré une théorisation du spirituel pratiqué, présentée dans cet article. Cette théorie du spirituel pratiqué qui émerge de la pratique des ISS est constituée de six dimensions : les rituels, l’appartenance, les valeurs, la transcendance, l’identité et le sens.

ZusammenfassungNoch immer besteht im medizinischen Kontext Unsicherheit im Verständnis der spirituellen Dimension von Krankheit und Schmerz und im Hinblick auf die Frage wie weit diese als eigene Dimension in das medizinische Menschenbild aufgenommen werden soll. Der Beitrag versucht, das Wesen spirituellen Leids auf dem Hintergrund gegenwärtiger neurobiologischer Erkenntnisse ausgehend von einer Analyse menschlicher Subjektivität zu klären. Menschliche Subjektivität und damit verbundene Fragen von Sinn und Bedeutung sind Bedingung für Spirituelles, sollten aber nicht selbst als „spirituell“ bezeichnet werden. Trotz einer feststellbaren terminologischen Verschiebung von „existentiellem Leid“ zu „spirituellem Schmerz“ sollte der Begriff „spirituell“ für Fragen stehen, die letzten Sinn oder letzte Transzendenz involvieren. Die existentielle und die spirituelle Ebene greifen trotzdem eng ineinander. In der Begegnung mit Kranken und Sterbenden sind terminologische Fragen im Vergleich zur Pflicht, alle faktisch bestehenden Formen von Leid und Schmerz wahrzunehmen und darauf zu antworten, jedoch sekundär. „Existentiell-spirituelles Leid“ könnte auf dieser Ebene ein geeigneter Begriff sein, um dem Ineinander von existentiellen Sinnfragen und spirituellen Fragen im engen Sinn gerecht zu werden.
