Introduction: Lichen planus (LP) is a chronic inflammatory dermatosis, histopathologically characterized by band like lymphohistiocytic infiltrate obscuring the dermo-epidermal interface and associated with basal cell vacuolar degeneration. Cell mediated cytotoxicity as evidenced by dermal T lymphocytes have been postulated to initiate or stimulate the pathogenic mechanisms for Lichenoid dermatitis. Aims and Objective: The aim of the study is 1) To determine the T cell subsets in the inflammatory population. 2) To analyze the pattern of distribution of T cells subsets in histopathologically diagnosed cases of LP. Materials and Method: Biopsies received from Dermatology department of Dr B R Ambedkar Medical College and histopathologically diagnosed as LP were subjected for immunohistochemistry for CD4 and CD8.Manual quantification and distribution of T cell subsets were analyzed by 2 individual observers. Result: Forty cases of LP diagnosed histopathologically were subjected for immunohistochemistry for CD4 and CD8. The mean age of the patients were 35.74 years with slight female predominance (F:M ratio 1.5:1). A mixed CD4 and CD8 phenotypes were identified in inflammatory population. At the dermoepidermal junction the mean CD8+ T cells subset was 67.5± 9.92% and mean CD4+ T cells was 32.5±9.92%. Perivascular quantification revealed a mean CD4+ T cells of 55±10.82 %and mean CD8+ T cells of 45±10.82%. Conclusion: In LP cases as CD8+ cytotoxic T cells density was more in D-E Junction compared to CD4+ T helper cells, a pathogenetic role of CD8+ T cells can be highlighted in basement membrane damage. An increase in perivascular CD 4+ T cells concentration signifies the pathogenetic role of dual T cell subsets in LP. This study would perhaps give a better insight to dermatologist to improvise the treatment response.

BackgroundRecently, several studies demonstrate the possible role of zinc oxide (ZnO) in the protection of several skin diseases, but less is known about the role of ZnO nanoparticles in the inflammatory skin disease. So, this study was designed to confirm the pivotal role of the nano zinc oxide cream in the alleviation of lead oxide (PbO) induced-allergic dermatitis in rats.Materials and methodsTwo concentrations (1% and 6%) of ZnONPs creams were prepared and characterized prior to being used in the study. A total number of 30 male Wistar rats were randomly divided into six groups. Group 1 (negative control), groups 2&3 (either 1% or 6% ZnONPs control groups), group 4 (PbO), groups 5&6 (co-treatment of each ZnONPs concentration+PbO). All rats in different groups were observed daily to determine the severity of dermal gross lesions. Histopathological studies, mRNA analysis, and oxidative stress evaluations were performed on the affected skin tissue. Immunohistochemical studies were performed to evaluate the expression of cluster of differentiation CD4, CD8 and intercellular adhesion molecules ICAM-1 in different groups.ResultsPbO caused extensive skin oxidative damage manifested by a significant increase in MDA level with a decrease in GSH content and CAT activity. The results of histopathological and immunohistochemical examinations revealed that topical application of PbO for 14 days led to severe allergic dermatitis with remarkable elevations in the number of CD4+ T-helper, CD8+ T-cytotoxic lymphocytes, and ICAM-1 expression. On the other hand, noticeable improvements were recorded in all the previous toxicopathological parameters among the groups treated by either 1% or 6% ZnO-NPs cream. However, the best results were observed in the group treated with 1% ZnO-NPs cream.ConclusionOur findings suggest that 1% of ZnO-NPs cream is safe when applied topically on the inflamed skin. Moreover, it had anti-inflammatory and antioxidant effects so that, it is recommended to use the 1% ZnO-NPs cream to avert the dermal toxicity-induced by PbO.

Clinical patient examinations are not enough for diagnosing lymphoproliferative skin diseases now. Histological diagnostics is an important auxiliary method; however, it is not always applicable. PCR is not a universal method for determining the clonality of infiltrate cells. The article describes present-day diagnostics methods with an emphasis on the immunohistochemistry method making it possible to identify the cell composition of affected skin.
