Oral Terbinafine and Itraconazole Treatments against Dermatophytes Appear Not to Favor the Establishment of Fusarium spp. in Nail

Abstract: Background: Fusarium onychomycoses are weakly responsive or unresponsive to standard onychomycosis treatments with oral terbinafine and itraconazole. Objective: To examine whether the use of terbinafine and itraconazole, which are highly effective in fighting Trichophyton onychomycoses, could be a cause of the high incidence of Fusarium nail infections. Methods: Polymerase chain reaction methods were used to detect both Fusarium spp. and Trichophyton spp. in nails of patients who had either received treatment … Show more

“…Frequencies vary between 1% and 16% [25,26]. Appropriate treatment is recommended because of the risk of dissemination from a local nail lesion upon increased immunosuppression [27]. The infections are clinically indistinguishable from onychomycosis by dermatophytes, and present as distal subungual, proximal subungual and total dystrophic infections [25,26].…”

“…Onychomycosis is difficult to treat, requiring generally, prolonged, systemic oral treatment with anti‐fungal agents due to lack of effective therapeutic topical preparations . The necessity of management by orally administered systemic drugs and prolonged periods of treatment may result in significant adverse effects . This may lead to problems of compliance, followed by low rates of therapeutic success and recurrences of infection.…”

“…A nested PCR assay is the modification of a conventional PCR approach in which the first amplification is followed by another amplification of a smaller region inside the initial amplified fragment [30]. The nested PCR consists of primary and secondary amplification to increase the specificity and sensitivity of a PCR [56] in which the primary PCR product is used as a template for secondary PCR to avoid primer dimer artefacts. Thus, it eliminates false‐positive and false‐negative results as well as discriminates between the specific and nonspecific DNA bands or, in other words, nested PCR tends to reduce the nonspecific binding in products due to the amplification of unexpected primer binding sites.…”
