A comparison has been made of the relative efficiency of Talampicillin and Minocycline given as single doses orally in the treatment of uncomplicated acute gonococcal urethritis in the male. One hundred and fifteen patients were given Talampicillin I.5Gm and of these II (9.6010) defaulted and were excluded. One hundred and thirty eight patients received 300mg Minocycline and of these 12 (8.7%) defaulted and were excluded. In the Talam-piciIIin group 8.7010 were presumed treatment failures whereas in the Minocycline group the figure was 3.2%.It is considered that single dose oral Talampicillin used alone gives disappointingly high failure rates in the treatment of acute gonorrhoea. Minocycline in single oral dosage remains a suitable and effective drug in the treatment of acute gonococcal urethritis. In this trial both drugs were free from side effects.

SUMMARY Two hundred and ten men suffering from uncomplicated gonococcal urethritis were treated with one of two treatment schedules. Of 109 patients treated with 3 g ampicillin and 2 g probenecid (group A) there were two recurrences in the first week after treatment and none in the second week. Of 101 patients treated with 1 5 g talampicillin and 2 g probenecid (group B), there were three recurrences in the first week and none in the second week. Both antibiotics were well tolerated, but one patient vomited two hours after taking talampicillin. The sensitivity pattern of Neisseria gonorrhoeae to penicillin at The London Hospital has shown an increase in the proportion of more sensitive strains during the last three years. Talampicillin given in a single dose with probenecid is satisfactory in the treatment of acute gonococcal urethritis but, apart from the smaller dosage, it does not offer any advantage over ampicillin with probenecid.

The rate of reinfection after treatment was studied in four groups of patients. The first group of 100 patients were evaluated retrospectively after treatment with 1.0 g ampicillin twice followed by two re-examinations over a 2-week period during which time intercourse was prohibited. The second and third prospectively groups consisted of 100 and 200 patients respectively, treated with 1 g ampicillin twice and 1.4 g pivampicillin + probenecid respectively, with re-examinations at intervals of 1 week and 4 weeks after treatment. In these two groups sexual intercourse was allowed during the last 10 days of the follow-up period. The fourth study group consisted of all patients infected with gonococci having MIC greater than or equal to 0.3 micrograms/ml for ampicillin and/or benzylpenicillin, who were treated with 1 g ampicillin twice during the period of 1973-76. In the first group no positive cultures were obtained at re-examination. In groups two and three, 35 out of 300 (12%) were found to have positive cultures at re-examination. Some of these were probably treatment failures. The failure rate in group four was found to be 20%, indicating that the treatment regimen used left a narrow therapeutic margin. However, the risk of re-infection, in this urban clinic, seems to be higher than the risk of treatment failure which would have remained undetected had sexual intercourse been forbidden.
