Oral Infections

The incidence of oral cancer is rising in the Sudan. Detecting oral cancer at an early stage is the most effective means of improving survival and reducing morbidity from disease. Aim:The aim of the present study was to determine the knowledge, attitude and practice of dentists related to oral cancer prevention and early detection in public dental clinics in Khartoum State, Sudan. Materials and Methods:A cross-sectional survey using an administered structured questionnaire was carried out. The questionnaire consisted of 28 questions that included the key areas of oral cancer prevention and early detection. A sample of 130 dental practitioners was requested to participate in the study. Results:The response rate was 86.92%. The majority were female (68.1%) between the ages of 20 -40 years old. The majority were aware of the major risk factors and the most common presentations of oral cancer. About two thirds (63.7%) perceived that their knowledge on oral cancer was current. However, almost a third (35.4.6%) reported that they had sufficient knowledge concerning oral cancer prevention. More than half (54 percent) reported that they do not carry out any special examination to detect oral cancer in asymptomatic patients. Two thirds (66.3%) believed that their undergraduate training in the diagnosis of oral cancer was inadequate. The vast majority (95.6%) were interested in further oral cancer continuing education and 88.5% that development of referral guidelines can improve the quality of referrals to specialists. Conclusions:The findings of the present study indicate that there is a need for further training in the early detection and prevention of oral cancer, as well as an increased emphasis on oral cancer diagnosis and prevention in the dental training curriculum.3 Declaration I, the undersigned, hereby declare that the work contained in this dissertation is my original work and that it has not been previously in its entirety or in part submitted at any university for a degree.………………………………… …………………………

Non-invasive label-free investigation and typing of head and neck cancers by multimodal nonlinear microscopy

Early detection and typing of tumors is pressing matter in clinical research with important impacts for prognosis and successful treatment. Currently, staining is the golden standard in histopathology but requires surgical removal of tissue. In order to avoid resection of non-diseased tissue a non-invasive real-time imaging method is required which can be applied ideally intrasurgically. In this proceeding a combination of second harmonic generation (SHG), two photon excited fluorescence (TPEF) and coherent anti-Stokes Raman (CARS) imaging has been employed to investigate tissue sections of head and neck carcinomas focussing on laryngeal carcinoma. Primary laryngeal and other head and neck carcinomas consist to 99% of squamous cell carcinoma. By fusing the various imaging methods it is possible to measure the thickness of the epithelial cell layer as a marker for dysplastic or cancerous tissue degradation and to differentiate keratinizing and nonkeratininzing squamous cell carcinomas (SCC). As nonkeratinizing SCCs of the oropharynx correlate with a human papillomavirus (HPV) infection as a subentity of head and neck cancer, and HPV related tumors are associated with a better clinical prognosis, the differentiation between keratinizing and non-keratinizing forms of SCCs is of high diagnostic value. TPEF is capable of displaying cell nuclei, therefore, morphologic information as cell density, cell to cytoplasm ratio, size and shape of cell nuclei can be obtained. SHG -on the other hand -selectively reveals the collagen matrix of the connective tissue, which is useful for determination of tumor-islets boundaries within epithelial tissue -a prerequisite for precise resection. Finally CARS in the CH-stretching region visualizes the lipid content of the tissue, which can be correlated with the dysplastic grade of the tissue.

Adult fibrosarcoma (FS), defined by the World Health Organization as 'malignant neoplasm composed of fibroblasts with variable collagen production and, in classical cases, a 'herringbone' architecture', is a very rare soft tissue sarcoma. The earlier literature revealed that the adult fibrosarcoma was the most commonly occurring lesion, however the incidence of which has declined dramatically over the past several decades. This is due to evolution in the classification of soft tissue tumors, recognition of its subtypes and increased understanding of the many other mesenchymal and nonmesenchymal tumors. Sarcomas are the rare group of malignancies which accounts for less than 1% of all the body sarcomas of which 5 to 15% occurs in head and neck region. It involves deep soft tissues of extremities, trunk, head and neck. Among the head and neck region, intraoral adult fibrosarcomas are very rare without any sex predilection. A case of primary soft tissue FS in posterior mandible of an 13-year-old female patient is documented here, which will be an addition to the cases reported earlier in the literatures.
