Abstract: Twins are at higher risk of suboptimal neonatal outcomes than singletons, but do better when delivered at early term rather than term or late term.

“…Data from all twin births that took place in the United States between 2007 and 2010 suggest that neonatal complications occurred less frequently in twins born between 37 and 38 weeks of gestation than in those born between 38 and 39 weeks [33]. Similar results were obtained in the analysis of more than 760 twin births in Canada [34].…”

“…If we are to consider the estimates that appreciate monochorionic twins as representing about a fourth (25%) of twin pregnancies, 12 our findings might be appreciated as biased towards and more representative for dichorionic pregnancies than monochorionic pregnancies. The increased risk of complications and iatrogenic preterm delivery in monochorionic twins is well known and documented in the research literature and practice guidelines.…”

“…7 Although several large epidemiologic studies were performed to determine an optimal delivery time for twin pregnancies, the results are still controversial, and a consensus has not been reached. 4,[7][8][9][10][11][12] Furthermore, most of these studies, using individual twins rather than twin pairs as the unit to calculate the mortality rate, may underestimate the risk of poor outcomes in twin pregnancies. Nonetheless, the optimal duration for pregnancy appears to be shorter in twins compared with singleton pregnancies.…”

“…Although there have been many studies performed to determine the optimal delivery time for twin pregnancy, the results are controversial and unsatisfying [ 12 13 ]. In this study, we analyzed the neonatal outcome according to gestational age at delivery in uncomplicated twin pregnancies based on the Korean data in order to determine the optimal time for delivery in uncomplicated monochorionic and dichorionic twin pregnancies.…”
