BACKGROUND:Melanocytic nevi excision is one of the commonly done procedure in dermatosurgery. Patient seeks excision of nevi for varied etiology like increasing in size, fear of malignancy, cultural beliefs and more commonly for cosmetic purpose. When it is done for cosmetic purpose, scar arising out of the procedure is a real concern for the patient. So this study was undertaken to evaluate the scar in patients who have undergone excision of melanocytic nevi using punch technique. AIMS: Subjective and objective analysis of the scar arising out of melanocytic nevi excision using punch. To study the recurrence rate of melanocytic nevi following punch excision. To evaluate the cosmetic outcome of the scar. SETTINGS AND DESIGN: A prospective study of evaluation of scar following punch excision of melanocytic nevi in 30 patients. MATERIALS AND METHODS: 30 patients who fulfilled the inclusion criteria were enrolled for the study. Punch excision technique was performed with prior consent and counseling. Patients were followed at day 3, and 5 post procedure and 6 th month for scar assessment. Both subjective and objective scar analysis was performed at the end of six months. RESULTS: Complete removal of nevi was achieved in all 30 patients with excellent cosmetic results with no recurrence at the end of 6 months. CONCLUSION: Melanocytic nevi excision with skin biopsy punch is a simple, inexpensive technique with excellent cosmetic outcome.

In this research, we present a semi-supervised segmentation solution using convolutional autoencoders to solve the problem of segmentation tasks having a small number of ground-truth images. We evaluate the proposed deep network architecture for the detection of nests of nevus cells in histopathological images of skin specimens is an important step in dermatopathology. The diagnostic criteria based on the degree of uniformity and symmetry of border irregularities are particularly vital in dermatopathology, in order to distinguish between benign and malignant skin lesions. However, to the best of our knowledge, it is the first described method to segment the nests region. The novelty of our approach is not only the area of research, but, furthermore, we address a problem with a small ground-truth dataset. We propose an effective computer-vision based deep learning tool that can perform the nests segmentation based on an autoencoder architecture with two learning steps. Experimental results verified the effectiveness of the proposed approach and its ability to segment nests areas with Dice similarity coefficient 0.81, sensitivity 0.76, and specificity 0.94, which is a state-of-the-art result.

Nevus melanositik intradermal merupakan varian nevus melanositik yang paling umum. Metode terbaik untuk terapinya adalah tindakan eksisi ellips. Bekas luka pasca operasi eksisi nevus memerlukan perhatian ekstra terutama pada area wajah karena pasien mengharapkan bekas luka yang minimal. Eksisi elips diorientasikan searah dengan relaxed skin tension line (RSTL) untuk mencapai hasil kosmetik yang optimal. Kasus pertama melaporkan seorang perempuan 23 tahun dengan tahi lalat di area dagu yang membesar mulai 15 tahun yang lalu. Keluhan gatal, nyeri, mudah berdarah pada tahi lalat disangkal. Pemeriksaan dermatologis pada regio mentalis didapatkan nodul hiperpigmentasi, soliter, oval, diameter ±6 mm, batas tegas, homogen, permukaan mengkilat dengan terdapat rambut, pada palpasi teraba keras dan solid. Kasus kedua melaporkan seorang perempuan 33 tahun dengan beberapa tahi lalat berwarna kehitaman di area hidung yang membesar sejak 27 tahun yang lalu. Keluhan gatal, nyeri, dan mudah berdarah pada tahi lalat disangkal. Pemeriksaan dermatologis pada regio nasal didapatkan papul hiperpigmentasi, multipel, solid, bulat, diameter ±2-4 mm, batas tegas, homogen, pada palpasi teraba kenyal. Histopatologis kedua kasus menunjukkan nevus intradermal. Pengangkatan nevus pada kedua kasus dengan bedah eksisi ellips pada pasien diikuti sampai 1 tahun pertama pasca operasi, dan didapatkan bekas luka yang minimal. Dapat disimpulkan bahwa pengangkatan nevus melanositik intradermal dengan bedah eksisi ellips merupakan salah satu modalitas terapi terbaik dalam pemeriksaan histopatologis untuk penegakan diagnosis. Pencegahan pembentukan bekas luka pasca operasi dapat diminimalisir dengan memperhatikan teknik eksisi, relaxed tension skin line, tehnik undermining, penjahitan, dan perawatan luka. Kata kunci: eksisi ellips, eksisi intradermal, nevus melanositik wajah.
