Optimal Combinations of Systematic Sextant and Laterally Directed Biopsies for the Detection of Prostate Cancer

Abstract: The standard sextant protocol failed to detect a large proportion of cancers located laterally in the peripheral zone. A 10 core biopsy regimen that combined laterally directed cores at the base, mid gland and apex of the prostate with mid lobar biopsy cores at the base and apex maximizes the sensitivity of transrectal ultrasound guided systematic biopsy.

“…As a matter of fact, even EPBx might miss up to 40% of PCas [13,14,15,16,17,18,19,20]. Additionally, about 10–20% of the PCas not found at initial PBx are located in the transition zone (TZ), that is, not sampled during the initial biopsy [21,22,23].…”

“…The optimal pattern in their study was a 10-core strategy with biopsies at the lateral base, mid, and apex, along with mid-sagittal biopsies at the base and apex. The highest cancer detection rates were at the lateral base and apex; the lowest yield came from TZ biopsies [25].…”

“…The overall cancer detection rate was 42%, with the highest detection rates observed with lateral and apex prostate biopsies [24]. Gore et al [25] systematically evaluated 396 consecutive patients undergoing TRUSguided sextant biopsies and extended lateral core biopsies. The authors evaluated all permutations of biopsy patterns to identify the minimum number and pattern of biopsies that would maintain a 98% cancer detection rate.…”
