Onset of exercise training 14 days after uncomplicated myocardial infarction: a randomized controlled trial

Abstract: An early start of exercise training did not increase VO2peak compared to 4 weeks of delay. For low-risk patients with high motivation for exercise training, home-based walking is an option as a moderate start of cardiac rehabilitation the first weeks after MI.

“…Our study provides insight regarding the views and perceptions of different stakeholders on a topic that is, both in daily practice and in national policies, often determined by non-empirical assessment among a small number of individuals. There is no objective standard for acceptable wait times, other than participation rates [3,4,18,19] and some emerging evidence of a relationship between wait time and peak oxygen consumption achieved [20,21] at this time, but our analysis of the subjective views of these stakeholders has provided important information regarding the appropriateness of the consensus recommendations.…”

“…Therefore, it is most important to improve cardiopulmonary function and prevent recurrence of myocardial infarction after AMI, and cardiac rehabilitation improves both effectively 3,4. Furthermore, as the period of hospitalization for myocardial infarction becomes shorter and many patients return to their daily life immediately after early discharge, several studies are investigating methods to maximize effects of cardiac rehabilitation programs by applying the programs as early as possible 12,13…”

“…Finally, 2 studies randomized patients to an early or later entry CR group. 42,43 Both studies excluded patients who had complications, such as heart failure, or were living far from the CR center. Macchi et al randomized 350 postvalve or CABG surgery patients to a 3-week CR program either early after surgery (mean of 7.8±2.7 days) or later (24.6±2.1 days).…”
