One-Shot Versus Metal Telescopic Dilation Technique for Tract Creation in Percutaneous Nephrolithotomy: Comparison of Safety and Efficacy

Abstract: One-shot dilation was proved to be safe and effective like metal telescopic dilation even in patients with a history of ipsilateral open renal surgery. In this procedure, X-ray exposure is lower.

“…Frattini et al have stated that, because of the development of retroperitoneal scar, dilatation with the one-shot technique generallyfails in patients with a history of open-stone surgery and, therefore, balloon or metal dilatation techniques are preferred in this patient group. In their study of 112 patients, Falahatkar et al examined one-shot technique by using a PVC dilator (Amplatz) and reported that nephrostomy access failed in three patients who had open-stone surgery; 9 they nevertheless commented that it was an effective and reliable method. However, access failure did not occur in any of the patients and no other dilatation method was needed.…”

“…From the first application of nephrostomy in 1955, developments in its technique, and equipment have improved patient outcomes. [12][13][14] Kukreja et al [15] demonstrated that during intrarenal access, the selection of the appropriate calyx had not any impact on the development of complications, on the contrary following optimal access with a needle the dilatation technique used had effected rates of bleeding. In 1994, Stoller et al [16] investigated the effects of telescopic metal, and balloon dilatation on blood loss, and couldn't find any significant difference between both methods.…”

“…However, shorter hospital stay has been demonstrated in the Amplatz group. [12,13,18,21,22] Higher cost of balloon dilators, disadvantages of their dynamic features, and single-use, need for Amplatz dilators when more than one access tract must be created for multiple, and staghorn stones demonstrate superiority of Amplatz dilators in terms of cost-effectiveness. [22,23] Since our study had a retrospective design, duration of dilatation, and radiation exposure were not evaluated.…”

“…We believe that one-step balloon dilation can reduce operation time [11,12,13], although the two-step method of tract dilation is popular in PCNL. In our series, the mean operative time was 72.8 min (range: 35-140).…”
