Oncologic reconstruction: General principles and techniques

Abstract: Halsted's principle of radical mastectomy influenced cancer treatment for decades. Randomized controlled trials resulted in a paradigm shift to less radical surgery and the use of adjuvant therapies. Oncologic reconstruction performed by plastic surgeons has evolved, ranging from skin grafts and local flaps for smaller defects to pedicled flaps and free flaps for larger and more complex defects. Immediate reconstruction facilitates resection is oncologically safe and contributes to meaningful improvements in q… Show more

“…Although time to complete epithelialization was greater than that reported for grafting, which is between seven and 14 days, it was an expected outcome, given that ADM is a dermal scaffold and lacks the dermal layers characteristic of autologous grafts. 1,5,11 Moreover, most of the study population was elderly, many of whom tolerated reconstruction under local anesthesia only. Our study demonstrated that resection of a cutaneous malignancy with ADM coverage was well tolerated in the elderly, even under local anesthesia only, although graft failure was more frequent.…”

“…P ATIENTS WITH CUTANEOUS malignancies have several options for reconstruction after surgical excision. 1 According to prior studies, 50 to 61 per cent of patients who underwent wide local excision for skin cancer required a reconstructive procedure for defect coverage. [2][3][4] For patients with multiple medical comorbidities, these reconstructive options become limited and often require complex planning for wound closure or coverage.…”

“…Pedicled skin flap [7,8] is heavily applied in oncologic and reconstructive surgery with the advantages of simplified procedure and reliable blood supply. However, in many cases, a giant defect or a large defect cannot be covered or covered properly with a certain single pedicled flap.…”
