On the characterization and uncertainty analysis of radiochromic film dosimetry

The purpose of this study was to investigate amplitude gating combined with a coached breathing strategy for 10 MV flattening filter‐free (FFF) volumetric‐modulated arc therapy (VMAT) on the Varian TrueBeam linac. Ten patient plans for VMAT SABR liver were created using the Eclipse treatment planning system (TPS). The verification plans were then transferred to a CT‐scanned Quasar phantom and delivered on a TrueBeam linac using a 10 MV FFF beam and Varian's real‐time position management (RPM) system for respiratory gating based on breathing amplitude. Breathing traces were acquired from ten patients using two kinds of breathing patterns: free breathing and an interrupted (~5 s pause) end of exhale coached breathing pattern. Ion chamber and Gafchromic film measurements were acquired for a gated delivery while the phantom moved under the described breathing patterns, as well as for a nongated stationary phantom delivery. The gate window was set to obtain a range of residual target motion from 2–5 mm. All gated deliveries on a moving phantom have been shown to be dosimetrically equivalent to the nongated deliveries on a static phantom, with differences in point dose measurements under 1% and average gamma 2%/2 mm agreement above 98.7%. Comparison with the treatment planning system also resulted in good agreement, with differences in point‐dose measurements under 2.5% and average gamma 3%/3 mm agreement of 97%. The use of a coached breathing pattern significantly increases the duty cycle, compared with free breathing, and allows for shorter treatment times. Patients' free‐breathing patterns contain considerable variability and, although dosimetric results for gated delivery may be acceptable, it is difficult to achieve efficient treatment delivery. A coached breathing pattern combined with a 5 mm amplitude gate, resulted in both high‐quality dose distributions and overall shortest gated beam delivery times.PACS number: 87.55.Qr

The purpose of this study is to validate Eclipse's electron Monte Carlo algorithm (eMC) in heterogeneous phantoms using radiochromic films and EGSnrc as a reference Monte Carlo algorithm. Four heterogeneous phantoms are used in this study. Radiochromic films are inserted in these phantoms, including in heterogeneous media, and the measured relative dose distributions are compared to eMC calculations. Phantoms A, B, and C contain 1D heterogeneities, built with layers of lung‐ (phantom A) and bone‐ (phantoms B and C) equivalent materials sandwiched in Plastic Water. Phantom D is a thorax‐anthropomorphic phantom with 2D lung heterogeneities. Electron beams of 6, 9, 12 and 18 MeV from a Varian Clinac 2100 are delivered to these phantoms with a 10×10 cm2 applicator. Monte Carlo simulations with an independent algorithm (EGSnrc) are also used as a reference tool for two purposes: (1) as a second validation of the eMC dose calculations, and (2) to calculate the stopping power ratio between radiochromic films and bone medium, when dose is measured inside the heterogeneity. Percent depth dose (PDD) film measurements and eMC calculations agree within 2% or 3 mm for phantom A, and within 3% or 3 mm for phantoms B and C for almost all beam energies. One exception is observed with phantom B and the 6 MeV, where measured PDDs and those calculated with eMC differ by up to 4 mm. Gamma analysis of the measured and calculated 2D dose distributions in phantom D agree with criteria of 3%, 3 mm for 9, 12, and 18 MeV beams, and criteria of 5%, 3 mm for the 6 MeV beam. Dose calculations in heterogeneous media with eMC agree within 3% or 3 mm with radiochromic film measurements. Six (6) MeV beams are not modeled as accurately as other beam energies. Sum signal dosimetry: A new approach for high dose quality assurance with Gafchromic EBT3

Gafchromic EBT3 film dosimetry in radiosurgery (RS) and hypofractionated radiotherapy (HRT) is complicated by the limited film accuracy at high fractional doses. The aim of this study is to develop and evaluate sum signal (SS) film dosimetry to increase dose resolution at high fractional doses, thus allowing for use of EBT3 for dose distribution verification of RS/HRT treatments. To characterize EBT3 dose–response, a calibration was performed in the dose range 0.44–26.43 Gy. Red (RC) and green (GC) channel net optical densities were linearly added to produce the SS. Dose resolution and overall accuracy of the dosimetric protocol were estimated and compared for SS,RC, and GC. A homemade Matlab software was developed to compare, in terms of gamma analysis, dose distributions delivered by a Cyberknife on EBT3 films to dose distributions calculated by the treatment planning system. The new SS and conventional single channel (SC) methods were compared, using 3%/1 and 4%/1 mm acceptance criteria, for 20 patient plans. Our analysis shows that the SS dose–response curve is characterized by a steeper trend in comparison with SC, with SS providing a higher dose resolution in the whole dose range investigated. Gamma analysis confirms that the percentage of points satisfying the agreement criteria is significantly higher for SS compared to SC: 95.03% vs. 88.41% (P = 0.014) for 3%/1 mm acceptance criteria and 97.24% vs. 93.58% (P = 0.048) for 4%/1 mm acceptance criteria. This study demonstrates that the SS approach is a new and effective method to improve dosimetric accuracy in the framework of the RS‐HRT patient‐specific quality assurance protocol.
