On-going generation of multiple forms of HIV-1 intersubtype recombinants in the Yunnan Province of China

Abstract: New recombinant strains are arising continually in west Yunnan near the Myanmar border. Some appeared to be secondary recombinants derived from CRF07_BC that had further recombined with other strains. The uneven distribution of subtypes, CRF and URF, suggests the presence of independent transmission networks and clusters among IDU in Yunnan.

“…Conversely, the HIV-1 strains circulating in the China-Myanmar border region showed a rapidly increased genetic diversity [3,5,6,33]. Pure HIV-1 subtypes, including B, C and CRF01_AE, were rarely detected among IDUs in this region; this increase in genetic diversity was accompanied by the rapidly increased prevalence of HIV-1 inter-subtype recombinants (greater than 87.3%) [3,4].…”

“…A large number of injecting drug users (IDUs) are active in this region and are associated with the cross-border transmission of human immunodeficiency virus type 1 (HIV-1) [1]. In the 1980s, the Thailand-original subtype B (Thai-B or B’), circulating recombinant form (CRF) 01_AE, and the India-original subtype C were introduced into this region, which led to the co-circulation of several HIV subtypes and to the ongoing generation of numerous recombinant HIV-1 strains [26]. Fourteen new CRFs have been identified in this region, including the two strains circulating nationwide in China, CRF07_BC and CRF08_BC [712].…”

“…Then from 1989 a number of infections arose in Chinese intravenous drug users from the Yunnan Province [4][5][6][7] and it appears this may be the point of common origin of the present day HIV epidemic in China. Thus the 2 decades of HIV prevalence in China was divided into three phases [8,9].…”

“…Around 1989, 146 HIV positive injecting drug users were identified in southwest of the Yunnan Province [4,[6][7][8]12]. During this period the large majority of all HIV cases identified in China were from this region.…”
