Ocular Surface Changes in Glaucomatous Patients Treated With and Without Preservatives Beta-Blockers

Abstract: In glaucomatous patients, preservative-free 0.1 timolol treatment improved their QoL. Similar dry eye signs or symptoms improved after 3 months of treatment reducing dryness, hyperemia, follicular hyperplasia, and foreign body sensation.

“…11 Different authors have shown that the lack of BAK in the drops can improve the ocular surface of glaucomatous patients. 15,24,25 Our results suggest that a single-dose tear supplement may reduce the damaging effects of the preservative BAK on conjunctival cells, on the tear film, and on sensory nerve fibers with alteration of the threshold of excitability, which follows a reduced sensitivity. Since corneal sensitivity and the number of nerve fibers in the cornea and BUT improved, a tear film supplement might be associated to a better corneal function.…”

“…The most likely mechanisms are corneal anaesthesia (Weissman & Asbell ), direct inhibition of adrenergic receptors (Petounis & Akritopoulos ) or ocular surface changes because of local toxicity from preservatives in non‐preservative‐free topical β‐blocker medications (Iester et al . ). Systemic β‐blockers have also been implicated in causing reduced tear production in both humans and experimental animals (Rahi et al .…”

“…Gözyaşı kırılma zamanı ve Schirmer testinde anlamlı değişiklik saptanmıştır. 14 Oküler yüzey belirtileri, hastanın yaşam kalitesini olumsuz etkilemekte ve tedaviye uyumunu azalmaktadır. Glokom ilaçlarına bağlı oküler yüzey hastalığı belirti ve bulgularının büyük bölümünün, aktif bileşenden çok prezervan maddeye bağlı olduğu çeşitli çalışmalarla kanıtlanmıştır.…”
