Occurrence of autoantibodies in chronic graft vs. host disease after allogeneic stem cell transplantation

Abstract: Chronic graft vs. host disease (GVHD) remains a major cause of morbidity and mortality after allogeneic stem cell transplantation (SCT). Chronic GVHD (cGVHD) has many similarities to de novo autoimmune disorders. While the presence and association of autoantibodies is well reported in these disorders, their role and clinical use remains a less studied area after SCT. We report the presence of autoantibodies in SCT recipients and a possible association with presence of cGVHD. During routine follow-up visits per… Show more

“…Consistent with these findings, the presence of antibodies specific for HY proteins were found to correlate with the subsequent development of chronic GVHD (18). Autoantibodies have also been described in patients with chronic GVHD (19,20). An immunopathologic role for B cells in this disease is further emphasized by the observation that treatment with anti-CD20 monoclonal antibody, rituximab, has resulted in clinical improvement in some patients with steroid-resistant chronic GVHD (21 -23).…”

“…In support of a B-cell role (4)(5)(6)(7)(8)(9), the presence of circulating autoantibody (10) and alloantibody (11,12) have been associated with development of cGVHD. Specifically, predominant B-cell subsets have been demonstrated in patients with cGVHD and identified in different studies as naïve (6) and postgerminal center B cells (7,8,10).…”

“…Chronic GVHD mimics or is associated with various autoimmune diseases (eg, systemic sclerosis, Sjögren syndrome, myasthenia gravis, and polymyositis). [9][10][11] Membranous nephropathy may represent another consequence of this graft-host conflict. 4 On the other hand, there are several case reports of membranous nephropathy after hematopoietic stem cell transplant without chronic GVHD; these suggest the existence of de novo membranous nephropathy after hematopoietic stem cell transplant.…”
