IntroductionPharmacological treatment may be effective for treating positive symptoms of schizophrenia; no evidence of clinically significant effects on negative and cognitive symptoms, social and behavioral functioning. This review investigated treatment outcomes of multiple (at least four sessions in 4 weeks) group music therapy sessions adjunct to standard care in inpatients with chronic schizophrenia.MethodsA systematic review search of five electronic medical and psychological databases conducted using keywords “music therapy” and “schizophrenia” up to December 2021. Screening was performed for published articles on any adjunct multiple group music therapy (four sessions in 4 weeks minimum) adjunct to “treatment as usual” for inpatients with “chronic” schizophrenia. All study outcomes were all included. Risk of bias of all studies was assessed.Results1160 articles were screened, and 13 randomized controlled trials (RCTs) with a total of 1,114 inpatients were included. Ten RCTs reported open group sessions with active structured music making (ASMM) combining passive music listening (PML) and/or active singing, playing instruments, and improvisations while three other studies applied PML only. Four studies reported significant outcomes for both positive and negative symptoms. Ten of the thirteen studies recorded significant improvements in negative symptoms, behavioral and social functioning. Lasting significant effects were found in a longitudinal RCT with 272 samples evaluated unguided pre-recorded PML as a coping method lasting up to six months and similar results found in another two longitudinal RCTs. Secondary outcomes measured cognition, mood, social interest and function, self-care ability, interpersonal relationships, and QoL all showed significant outcomes. The significance level for pre-post intervention and between-group measures ranged from p < 0.001 to p < 0.05. No negative effects were reported in any studies.ConclusionEvidence from this review suggests rehabilitation with adjunctive regular PML or combined ASMM in group settings may provide therapeutic engagement, contributing to improvements in social interest and participation. PML is low-cost and non-invasive therapy. Enhancing overall QoL as one type of psychosocial therapy. More rigorous longitudinal studies with larger sample sizes are needed to investigate whether regular long-term individual PML and active group music therapy have the same significant treatment effects as coping and rehabilitation strategies.

Background: Music-based interventions are currently being researched for their effectiveness in schizophrenia. Very few studies have been done in the acute phase. Further, there is a dearth of published Indian research in this context. Methods: A prospective, randomized, controlled, single-blind study was done in a tertiary psychiatric hospital in South India from October-2015 to March-2016. Immediately after admission, consenting adult patients with schizophrenia were randomly assigned to two groups. Over four weeks, Group-A(N=51) received 20 sessions of music-listening as an add-on to standard care, and Group-B(N=53) received standard care alone. Reduction in Positive and Negative Syndrome Scale (PANSS) scores and the drug dosage requirements for both groups were compared. Statistical analysis was done using two-way repeated-measures ANOVA for PANSS score reduction and Mann-Whitney U for drug dosage comparison. Results: We observed a greater decrease in PANSS scores in Group-A than Group B, but this difference did not reach significance (p>0.05 for all three subscales and total score). Group-A required lesser risperidone-equivalent doses than Group-B, but this was not significant (p=0.27). However, Group-A required significantly lesser diazepam equivalent (p=0.003) and trihexyphenidyl (p=0.007) doses than Group-B. Conclusions: The above findings suggest a possible additional benefit of adding music listening to standard care for the acute phase management of schizophrenia and reducing drug dosage requirements. Music-based interventions for schizophrenia warrant further research, especially in the Indian context and wider application in clinical practice.

Las investigaciones en el campo de la aplicación de la terapia musical como herramienta terapéutica han aumentado en los últimos diez años. Las revisiones narrativas publicadas hasta la fecha muestran los efectos y la eficacia de la terapia musical como estrategia de intervención rehabilitadora. Sin embargo, no tratan la temática desde una perspectiva general y ninguna de ellas ha incluido el término “musical therapy” entre los términos de búsqueda de literatura, quedando excluidos, por tanto, los resultados de toda una serie de estudios que han contribuido a esclarecer la eficacia de la musicoterapia en rehabilitación. Por estas razones, el objetivo de este trabajo es remediar esta laguna y revisar los hallazgos de investigaciones publicadas en los últimos diez años que han quedado excluidas de revisiones anteriores. Específicamente, esta contribución pretende responder a las preguntas de investigación: ¿Qué tipo de trastornos se pueden beneficiar eficazmente de la musicoterapia? y ¿Qué efectos positivos tiene la musicoterapia en rehabilitación? Para responder a estas preguntas, se llevó a cabo una búsqueda electrónica de literatura en las bases de datos Academic Search Complete, Medline y Science Direct. La búsqueda incluyó el término “musical therapy” y ciertos criterios de inclusión, tales como los reportes sobre estudios empíricos redactados en que utilizaran música como estimulación terapéutica. Como resultado, se revisaron en total veinticuatro estudios, de los que se analizaron una serie de elementos, tales como los grupos de participantes (experimental vs. control), los resultados obtenidos y las limitaciones. Como conclusión, afirmamos, con cautela, que la musicoterapia puede contribuir a la rehabilitación de las funciones cognitivas, motoras y sensoriales del daño cerebral; a la rehabilitación de la esquizofrenia y de la depresión primaria; a la mejoría de los trastornos neurodegenerativos, de los trastornos del espectro autista, del abuso de sustancias y de otras patologías. Para el futuro, recomendamos superar las limitaciones metodológicas de estos estudios y la ejecución de estudios transculturales.
