Objective: Detailed examination of occupational and environmental exposure of the individuals and their use of cigarette during history-taking stage in Chest Diseases is crucial. This study was planned to investigate the qualities of occupational anamneses carried out in the Outpatient Clinic of Chest Diseases. Methods:The recordings of 320 consecutive outpatients (158 female and 162 male) admitted to the Outpatient Clinic of Chest Diseases in the Faculty of Medicine were evaluated retrospectively. During this evaluation, whether occupational histories of the patients had been taken in detail or not and the manner in which questioning was performed regarding environmental exposure and smoking were assessed. Results:The mean age was 51.6±17.3 years in female patients and 55.1±16.1 years in male patients. The most common occupations were housewife (37.5%), farmer (12.8%) and worker (12.2%). Detailed occupational history-taking rate was 21.9% and was found to be higher in males (79%) than in females (31%) (p=0.001). Of the workers, 60% had been questioned regarding their current workplaces, while only 10.6% had been questioned about their previous workplaces. In total, 9 patients (2.8%) had been asked about the environment in which they live. The smoking habits had been questioned in 90.6% of the cases. Of the 290 patients who had been smoking, 183 (63.1%) had been questioned about the amount of cigarettes smoked in a year (packs.year). Conclusion:It was observed that extensive occupational anamnesis and a history of environmental exposure and smoking were not taken adequately in the Outpatient Clinic of Chest Diseases.

A relação entre as exposições ocupacionais e o aparecimento de doenças já é conhecida desde a antiguidade. A primeira publicação descrevendo a associação entre o trabalho de mineração e a manifestação de doença respiratória data de 1556, a obra De Re metállica, de Georg Bauer.(1) Bernardino Ramazzini, considerado o pai da medicina do trabalho, publicou na Itália, em 1700, o livro De morbis artificum diatriba, no qual descreve, com extrema clareza e perfeição, 54 doenças relacionadas ao trabalho e introduz na anamnese clínica uma simples pergunta: "Qual é a sua ocupação?". Desde então, inúmeros estudos foram realizados evidenciando essa relação causal tão expressiva e ainda hoje pouco valorizada em nossas anamneses. Essa evidência causal pode ser melhor compreendida se considerarmos que o aparelho respiratório, com cerca de 100 metros quadrados de superfície de contato, constitui a principal interface entre o nosso meio interno e o externo e, sabendo-se que um adulto inala entre 7 a 12 mil litros de ar por dia, contendo milhares de agentes aerodispersóides, podemos estimar as repercussões deletérias decorrentes dessas interações. RESUMODa mesma forma que uma boa anamnese é fundamental para o diagnóstico em geral, a história de exposição ambiental e ocupacional é informação basilar para o entendimento das doenças respiratórias ocupacionais. Estima-se que até 20% das doenças intersticiais e das vias aéreas são decorrentes dessas exposições. Por outro lado, raramente encontramos na anamnese informações sobre eventuais exposições ambientais ou sobre o trabalho/ocupação/função dos nossos pacientes. Assim, esse tema torna-se imprescindível à discussão entre os profissionais de saúde envolvidos nesse campo da atuação médica, se realmente almejamos avaliar a estimativa da magnitude desse problema em nosso meio.Descritores: Doenças ocupacionais/história; Exposição ambiental; Doenças respiratórias; Pneumopatias; Condições de trabalho; Exposição ocupacional ABSTRACTJust as a complete patient history is fundamental to making any diagnosis, the history of environmental exposure in the workplace forms the informational base needed in order to understand occupational respiratory diseases. It is estimated that 20% of all cases of interstitial or airway disease are related to such exposure. However, information regarding potential environmental exposure is rarely included in the patient history, nor is job/occupation/function taken into consideration. Therefore, this theme has become pivotal to the discussion among health care professionals working in this area, who must decide whether evaluating and estimating the magnitude of this problem are truly desirable goals.

La Historia Ocupacional es una parte de la Historia Clínica del paciente que documenta las condiciones de salubridad de los puestos de trabajo desempeñados por él y relaciona la existencia de riesgos ocasionados o agravados por el trabajo que puedan devenir en enfermedades ocupacionales. Es una condición “sine qua non” la exposición a determinado riesgo para que sea posible la existencia de una enfermedad ocupacional. Para ello es imprescindible la consignación metódica y completa de la vida laboral del paciente que reconozca los posibles riesgos en relación al número de contaminantes y al tiempo (en años) de exposición a cada uno de los mismos. Por lo tanto, esta se constituye en una herramienta diagnóstica propia del médico especialista en Medicina Ocupacional.
