Existe una conveniencia en describir la Historia no a través de los sucesos culturales sino a través de la evolución del trabajo, dado su rol en la creación y mantenimiento de las sociedades humanas. Entonces, la preocupación de la medicina por la salud de los trabajadores reivindicaría su papel en el desarrollo de la humanidad. Sin embargo, como todo conocimiento está sujeto a diversificaciones en su contenido, tanto en la práctica como en el conocimiento, creando diversos conceptos que van evolucionando en el tiempo. Esto ha ocurrido con la Medicina del Trabajo ahora llamada Medicina Ocupacional y del Medio Ambiente, que es una disciplina fundamental dentro de la Salud Ocupacional. El presente artículo trata de esclarecer conceptos de la Medicina del Trabajo, la Medicina Ocupacional y del Medio Ambiente y la Salud Ocupacional.
El ámbito de la práctica de la medicina ocupacional ha sufrido cambios importantes en este último siglo, como consecuencia de las expectativas de empleadores y trabajadores, así como de la sociedad y el Gobierno. El papel del médico ocupacional se ha ampliado desde la identificación de las enfermedades ocupacionales hasta la evaluación de la productividad del trabajador referido al absentismo y el presentismo. Las actividades están orientadas a la salud del trabajador, su familia y su comunidad. El énfasis está sobre intervenciones preventivas y políticas más que el tratamiento. El campo de acción de la medicina ocupacional también se ha ampliado desde la práctica en plantas industriales a las universidades, hospitales, clínicas, así como consultorías privadas y del Gobierno. Finalmente, la salud y la productividad son un componente importante de la Salud Ocupacional y Pública, la Seguridad, el manejo empresarial de las pérdidas, evaluación de menoscabo o incapacidad, la promoción de salud, control de materiales peligrosos, etc.
La Historia Ocupacional es una parte de la Historia Clínica del paciente que documenta las condiciones de salubridad de los puestos de trabajo desempeñados por él y relaciona la existencia de riesgos ocasionados o agravados por el trabajo que puedan devenir en enfermedades ocupacionales. Es una condición “sine qua non” la exposición a determinado riesgo para que sea posible la existencia de una enfermedad ocupacional. Para ello es imprescindible la consignación metódica y completa de la vida laboral del paciente que reconozca los posibles riesgos en relación al número de contaminantes y al tiempo (en años) de exposición a cada uno de los mismos. Por lo tanto, esta se constituye en una herramienta diagnóstica propia del médico especialista en Medicina Ocupacional.
La principal causa de absentismo en el trabajo, es la incapacidad derivada de enfermedades, incluyendo a los accidentes de trabajo y particulares, las cuales pueden constituir hasta las tres cuartas partes del absentismo en la industria. Objetivo: Analizar las causas de absentismo laboral de origen médico en una empresa minera. Material y Métodos: Se realizó un estudio retrospectivo, que consistió en la revisión de los descansos médicos en el año 2001, realizados en el Hospital de Toquepala SPCC, localizado a 3,600 msnm. Se revisaron 541 descansos médicos, que basan sus diagnósticos en el sistema de Clasificación Internacional de Enfermedades de la Organización Mundial para la Salud (CIE – 10). Para las definiciones de los indicadores se siguieron las recomendaciones del Comité Permanente y la Asociación Internacional para la Medicina del Trabajo. Resultados: Las causas médicas de absentismo laboral fueron, por enfermedad común 445 (82.26%), accidentes 89 (16.45%) y maternidad 7 (1.29%). Entre las causas de enfermedades comunes tenemos que los descansos médicos por procesos infecciosos fueron 158 (29.20%), siendo de origen respiratorio 88 (16.27%), digestivo 50 (9.24%) y dermatológicas 20 (3.67%); y las causas no infecciosas 287 (53%). Los índices en nuestra población fueron: Indice de Frecuencia de 0.45; Indice de gravedad de 5.41, y duración media de ausencias de 11.81. Sin embargo, la literatura actual establece que una mejor evaluación de los índices debe ser hecha en base al número hombre-horas trabajada, de éste modo nuestros índices fueron: Indice de Frecuencia de 0.16 y el Indice de Severidad de 1.87. Conclusiones: Concluimos que nuestra empresa presenta bajos índices de absentabilidad laboral de causa médica, pero que debe intensificarse los programas de salud para la prevención de enfermedades infecciosas.
Se atribuye al Síndrome de Burnout o de Agotamiento Profesional, como el responsable de la desmotivación que sufren los profesionales sanitarios. En nuestra realidad, poco se ha investigado y escrito sobre el Síndrome de Burnout, sin embargo, creemos que su presencia en el personal sanitario está presente y en incremento. Objetivo: El presente estudio pretende identificar los trabajadores sanitarios con Síndrome de Burnout en nuestro centro de labor y aportar las características epidemiológicas que permitan una aproximación del perfil de riesgo en el personal sanitario. Materiales y métodos: Se entregó el Maslach Burnout Inventory al personal sanitario de los hospitales de una empresa minera del sur de Perú, previa explicación del estudio. Resultados: Se recibieron 82 (63%) cuestionarios debidamente llenados de los 130 entregados, de los cuales 26 fueron médico(a)s, 35 enfermero(a)s y 21 auxiliares de enfermería. Solo un médico presentó puntaje alto para cansancio emocional (CE) y despersonalización (DP) y bajo para baja realización personal (BRP). Conclusiones: En la población estudiada encontramos un médico con diagnóstico de Síndrome de Burnout, quien se caracterizó por el desarrollo temprano del síndrome y no asociado a la realización de turnos. Además concluimos en la necesidad de repetir el estudio en instituciones de salud públicas.
