ZusammenfassungZwanghaftes Zweifeln (zZ) ist ein eher vergessenes und unterschätztes psychopathologisches Phänomen, obgleich er klinisch bei vielen Patienten mit einer Zwangsstörung vorhanden zu sein scheint. Die wenige vorhandene vor allem psychoanalytische und kognitionswissenschaftliche Literatur zum zZ erklärt das Phänomen nicht vollständig und bietet darüber hinaus nur wenig an Behandlungsperspektiven. Hier soll ein anderer Weg vorgeschlagen werden. Das dem Menschen gehörige Zweifeln an der Wahrnehmung von Objekten, Anderen und sich selbst steigert sich als zZ ins Pathologische. Zweifeln wird als ein existenzielles Problem des Menschen angesehen, das als solches zu den Grundfragen der Philosophie seit ihren Anfängen, speziell zum Skeptizismus gehört. Wenn bzgl. dieses Zweifelns Gegengründe gefunden werden könnten, würden sich für die Behandlung von Patienten mit zZ mentale Lösungsstrategien im Rahmen theoretisch vertiefter Psychotherapie einschließlich kognitiver Übungsansätze ergeben.

The author attempts to understand the underpinnings of a ruminative depression that occurred in a patient after a troubled first analysis. Negative therapeutic reaction is often assumed to be the result of a patient's unconscious guilt or masochism and thus an intrapsychic phenomenon, but the author asserts that iatrogenic phenomena in the form of persistent misunderstandings and enactments that remain unanalyzed contribute to a destructive treatment experience. The analysand may relive the failed treatment again and again in his or her mind in an attempt to resolve it. The author asserts that a traumatic treatment experience can foster depressive rumination.

The paper describes an analysis with a very difficult, emotionally inaccessible patient who, after more than a year, unexpectedly terminated the analysis, leaving the analyst with a sense of frustration due to incomplete and interrupted analysis. In the theoretical section of the paper, the author presents his understanding of the creation of such a pathological organization, starting with Freud's concept of death drive, then going on to Melanie Klein's and her successor's discoveries. John Steiner's concept of 'psychic retreat' helps us to understand the process of development of a pathological organization of personality. In the last part, the author describes how he works through mourning and prompts reflection on the reasons for the premature termination of the treatment by the patient. He indicates the causes in both the patient and the analyst.
