Obesity Is Associated With Increased Morbidity in Moderate to Severe COPD

Abstract: the COPDGene Investigators BACKGROUND: Obesity is prevalent in the United States; however, the impact of obesity on COPD morbidity is unclear. We hypothesized that obesity is associated with worse outcomes in COPD.

“…However, the impact of increased body weight on the outcomes of COPD was not conclusive. Cao et al 7 conducted a meta-analysis of 22 studies with 21,150 participants and concluded that overweight and obese COPD patients were associated with a lower risk of mortality (risk ratio [RR] 0.47, 95% CI 0.33-0.68, and 12 found that obesity was associated with worse COPD-related outcomes, including HRQL, dyspnea (mMRC score) during a 6-minute walk test, and severe exacerbations of COPD.…”

“…Some studies have reported that overweight or obesity had a protective effect against mortality in patients with COPD, 6,7,9 while others have reported a worse HRQL, increased severity of dyspnea and frequency of severe exacerbations in obese patients with COPD. [10][11][12] To the best of our knowledge, few studies have explored the relationship between overweight or obesity and the frequency of COPD exacerbations. [12][13][14][15] Therefore, the aim of this study was to examine whether overweight and obesity are associated with the frequency of acute exacerbations in patients with COPD in Taiwan.…”

“…In the above study, the association between obesity and worse outcomes was independent of the presence of comorbidities and was associated, in a dose-dependent manner, with worse QoL, dyspnea, endurance to six minutes walking distance (6 MWD) and severe acute exacerbation of COPD (AECOPD). These associations were strengthened when obesity was analyzed as a dose-dependent response [29]. On the other hand, in a not fully understood reason, obesity seems associated with a reduced mortality risk in COPD patients (so-called “COPD–obesity paradox”) [30].…”

“…Так, согласно исследованиям Национального центра по изучению здоровья и пита-ния (США) за период 2007-2010 гг. по сравнению с 90-ми годами прошлого столетия, в качестве основной коморбидности больных с ХОБЛ уже рассматривается ожирение [11], в то же время доказано, что ожирение ухудшает прогноз больных с ХОБЛ [12].…”
