Aim: To explore the relationship between the practice environment and nurse outcomes in two Intensive Care Unit (ICU) models. Background: Internationally the demand for intensive care is increasing. A large capacity multi-specialty integrated critical care service, the "hot-floor", is emerging as the preferred organisational model. Benefits include resource consolidation and improved utilisation, operational synergies, operational flexibility and demand management. A large clinical workforce with commensurate frontline management, education and support positions are required. The association between these factors, within the ICU hot-floor work environment, and nurse outcomes is not known. Methods: Registered nurses (RNs) working in two ICUs, one a hot-floor model and one traditional ICU, completed a structured questionnaire. Nurse perceptions of work-life and organisational factors, and dimensions of burnout were examined using the Practice Environment Scale-Nursing Work Index (PES-NWI) and Maslach's Burnout Inventory (MBI). Results: Units matched on service level characteristics, training accreditation, patient casemix, operational and clinical care processes. Nurses in had similar demographic characteristics, professional attributes and experience. Workforce structures were also similar though the hot-floor had relatively less dedicated resources for frontline nurse management and clinical education positions. Hot-floor nurses worked more paid overtime and were redeployed less frequently to external wards. Nurse manager leadership and support was less effective, and nurses expressed lower personal accomplishment. Conclusions: Improved demand management achieved through greater operational flexibility is a key driver for the hot-floor model. Planning for enhanced organisational effectiveness requires corresponding improvements in the work environment to optimise nurse retention to ensure organisational sustainability.

RESUMO: Este estudo tem como objetivo identificar a influência das cargas de trabalho na saúde do trabalhador de enfermagem. A busca foi realizada em novembro de 2016, em quatro bases de dados, estabelecendo-se critérios de inclusão e exclusão, sendo selecionados 50 estudos. Os resultados destacam os conceitos, tipos e condições de trabalho que predispõem as cargas de trabalho, evidenciando as consequências na saúde da equipe de enfermagem por meio dos adoecimentos, que acarretam afastamentos e absenteísmo, estresse, desgastes físicos e emocionais, riscos para acidentes de trabalho, entre outros. Conclui-se que a identificação dessas influências possibilita a implementação de ações organizacionais, de prevenção de acidentes e doenças relacionadas ao trabalho. The search was conducted in November 2016, at four databases, and inclusion and exclusion criteria were established. At the end, 50 studies were selected. The results highlight work concepts, types and conditions, demonstrating their effects on the health status of the nursing team, which is manifested through illnesses that result in temporary absence from work (sick leaves) and absenteeism, stress, physical and emotional fatigue, risks of occupational accidents, among others. It is concluded that the identification of these effects allows the implementation of organizational actions targeted to the prevention of occupational accidents and diseases. Se destacan los conceptos, tipos y condiciones de trabajo que predisponen las cargas de trabajo, evidenciando las consecuencias en la salud del equipo de enfermería por medio de las enfermedades, las cuales resultan alejamiento y absentismo, estrés, desgastes físicos y emocionales, riesgos para accidentes de trabajo, entre otros. The Impact of Demographic Features and Environmental Conditions on Rates of Nursing Burnout

Background: The burnout phenomenon is a complex and multidimensional reality and also a common metaphor for a state of extreme psychophysical exhaustion that is usually related to work. Objectives: The goal of this study was to evaluate nursing burnout in 371 registered nurses at both governmental and private hospitals in Shiraz, Iran. Patients and Methods: This was a cross-sectional study that evaluated nursing burnout using the Maslach Burnout Inventory. Independent sample T-test and one-way analysis of variance were performed to determine the association between demographic factors, the nurses' working environment, and nurse burnout. Results:The results of this study showed that nurses in internal medicine wards, at staff positions, and on rotating shifts in governmental hospitals have a high rate of total burnout (P < 0.05). In contrast, nurses with less than five years of work experience have a high rate of reduced personal accomplishment (P < 0.05). Also, nurses with 6 -15 years of work experience reported the highest incidence of emotional exhaustion (P < 0.05). Conclusions: Burnout is associated with some specific demographic and environmental features. To reduce and prevent burnout, nursing leadership should focus on personal accomplishments in nurses with little work experience, especially for nurses in internal medicine wards, on rotating shifts, and on staff wards. In addition, policy makers at higher levels should also consider nursing burnout in governmental hospitals.
