Number of fragments, margin status and thermal artifacts of conized specimens from LLETZ surgery to treat cervical intraepithelial neoplasia

Abstract: CONTEXT AND OBJECTIVE: Large loop excision of the transformation zone (LLETZ) is a nontraumatic cut and coagulation method with several advantages, but it induces thermal artifacts in the cut region. The aim here was to assess the correlations of age, number of fragments, lesion grade and degree of thermal artifacts with margin quality in conized specimens from LLETZ for cervical intraepithelial neoplasia (CIN). DESIGN AND SETTING: Cross-sectional study at Universidade Federal de São Paulo (Unifesp). METHODS:T… Show more

“…The most compromised margins in our study were endocervical margins (80%), as in other studies in the literature 13 . Although the finding of endocervical positive margins is of greater concern, it need not necessarily confirm the presence of residual disease 10,13,14 .…”

“…Bittencourt et al reported that severe artifacts occurred in 22.8% of their patients. 23 The hybrid use of cold and electric knives in our method was designed to address the thermal artifact caused by the energy device and its adverse effect on pathological interpretation. The cold knife was used at the external and internal margin of the specimen of conization.…”

“…The difficulties in margin assessment due to the presence of artifacts are also de-scribed, particularly from the pathologistsʼ point of view [8]. Surgical textbooks and publications often underline the significant influence of surgical experience on surgery results [6,14]. Nevertheless, surgical experience is not the only determining factor of LLETZ specimen quality.…”

“…Based on the recommendations of the National Health Service Cervical Screening Programme from the United Kingdom, at least 80 % of the LLETZ procedures should result in single piece specimen, which is a marker of good practice [23]. Still, the rate of single fragment LLETZ cones in literature ranges from 49 to 92% [2,6]. The trainee status in colposcopy was previously established to be a substantial risk factor for specimen fragmentation [3].…”
