The occurrence of hospital-acquired acute gastroenteritis (AGE) is a major concern for public health. RotavirusA (RVA) and norovirus (NoV) are common causes of viral AGE in the pediatric population, and their role in nosocomial infections has been proven, remaining poorly investigated. To investigate RVA and NoV in hospital-acquired AGE, 55 stool samples from children with nosocomial AGE were collected between May 2014 and May 2015. To evaluate virus spreading routes, 51 environmental swabs were collected from staff and patients' rooms. Stools were tested for both RVA and NoV RNA by reverse-transcription-PCR. Nucleotide sequencing and phylogenetic analysis were performed to characterize the viruses. Forty-seven of 55 cases analyzed resulted positive for RVA. The predominant genotype was G4P[8] (18/55) followed by G1P[8] (14/55). Mixed RVA infections were also detected (7/55). Twenty-two samples were positive for NoV, and GII.4 was revealed to be the predominant genotype. Seventeen samples were positive for both RVA and NoV. This study aimed to evaluate the burden of norovirus and rotavirus nosocomial AGE, contributing to identify the environment source of infections and to activate effective strategies for intervention. The reduction in nosocomial AGE cases is an important aspect, considered the worsened disease course in transplant, cancer, and intensive care unit inpatients.

Re–evaluation of the cost–effectiveness and effects of childhood rotavirus vaccination in Norway

BackgroundRotavirus vaccination was included into the Norwegian childhood immunisation programme in 2014. Before implementation, rotavirus vaccination was found to be cost–effective from a societal perspective, but not from a healthcare perspective. Since introduction, new data on the incidence and economic effects of rotavirus disease have become available. We assessed early epidemiological effects of the rotavirus vaccination programme and re–evaluated its cost–effectiveness in Norway for the years 2015–2019.MethodsUsing a dynamic transmission model, we compared the epidemiological effects of the ongoing two–dose vaccination programme with Rotarix®, and a hypothetical 3–dose programme with RotaTeq® with no vaccination. A baseline cost of € 54 per fully vaccinated child was used. Cost–effectiveness was computed from a healthcare and societal perspective, using a decision analytical model. Data on healthcare use and costs, productivity losses and health utilities were based on published and own estimates. Uncertainty was accounted for in one–way, multi–way, and probabilistic sensitivity analyses.ResultsDuring 2015–2019, 114,658 home care cases, 34,571 primary care cases, 7,381 severe cases, and 2 deaths associated with rotavirus disease were avoided due to vaccination. Under baseline assumptions vaccination was cost–effective from a healthcare perspective with a cost per QALY of € 47,447 for Rotarix® and € 52,709 for RotaTeq®. The break–even price was € 70 for Rotarix® and € 67 for RotaTeq®. Vaccination was cost–saving from the societal perspective, and also from a healthcare perspective for vaccine prices below € 25 and € 22 per vaccinated child for Rotarix® and RotaTeq®, respectively.ConclusionOngoing childhood rotavirus vaccination in Norway has reduced the rotavirus disease burden substantially, and is cost–effective compared with no vaccination.

Ротавірусна інфекція посідає особливе місце в структурі захворюваності дітей як в Україні, так і в усьому світі, викликаючи тяжкі дегідратуючі гастроентерити переважно в дітей віком до 5 років. Практично кожна дитина до п'ятирічного віку переносить ротавірусну інфекцію незалежно від раси та соціального економічного статусу. А це робить таку інфекцією актуальною проблемою сучасної педіатрії та диктує необхідність її поглибленого вивчення. Мета-вивчити клініко лабораторні особливості перебігу ротавірусної інфекції в дітей раннього віку; визначити фактори, що впливають на його тяжкість. Матеріали та методи. Проаналізовано перебіг ротавірусної інфекції в 57 дітей віком від 1 міс. до 24 міс., які перебували на стаціонарному лікуванні в КНП «Обласна інфекційна клінічна лікарня» Запорізької обласної ради. Тяжкість перебігу ротавірусного гастроентериту визначено за шкалою Везикарі та виражено в балах (від 1 до 20). Результати. Діти віком від 12 міс. до 24 міс. становили більшість досліджуваної групи-47,3%, достовірно перевищуючи частку дітей віком до 6 міс.-22,8% (р=0,01). У більшості (70,2%) хворих ротавірусна інфекція перебігала в тяжкій формі із сумарною кількістю 15,00 [13,00; 16,00] балів за шкалою Везикарі. 52,6% дітей мали ускладнення хвороби, серед яких домінував вторинний ацетонемічний синдром. Захворювання достовірно тяжче перебігало в дітей віком від 12 міс. до 24 міс., сумарний показник балів у яких становив 13,5 [12,00; 15,00], проти 10,00 [8,00; 15,00] у дітей віком до 6 міс. (р=0,04) та 12 [10,00; 15,00] у хворих від 6 до 12 міс. Клінічними параметрами, що оцінювалися як тяжкі за шкалою Везикарі та обумовлювали тяжкий перебіг хвороби, були тривалість діареї (>6 діб-82,14% дітей), максимальна частота випорожнень на добу (>6 разів-69,6% дітей) та гіпертермія (піретична температура тіла-45,8% дітей). Серед коморбідної патології у хворих при тяжкому перебігу ротавірусної інфекції достовірно частіше, ніж у дітей із нетяжкою її формою, виявляли харчову алергію та атопічний дерматит-відповідно у 40% і 11,8% дітей (р<0,05). Висновки. Ротавірусна інфекція перебігає в тяжкій формі у дітей раннього віку та в більшості випадків має ускладнення. Предикторами її тяжкого перебігу є вік дітей від 12 міс. до 24 міс. і наявність в анамнезі харчової алергії та атопічного дерматиту. Дослідження виконано відповідно до принципів Гельсінської Декларації. Протокол дослідження ухвалено Локальним етичним комітетом зазначеної в роботі установи. На проведення досліджень отримано інформовану згоду батьків дітей. Автори заявляють про відсутність конфлікту інтересів. Ключові слова: ротавірусна інфекція, діти раннього віку, шкала Везкарі.
