An important concept in patellofemoral joint function is the quadriceps angle (Q angle), but there is a standardization problem with the measurement method. The aim of the study was to compare the change of the Q angle in relaxed or maximum voluntary contraction (MVC) of the quadriceps using a standardized measurement method in patients with patellofemoral pain syndrome (PFPS). This cross-sectional analytical study includes 34 sedentary female patients aged 18-50 years with a diagnosis of PFPS. Measurements were carried out with a manual extendable fixedarm goniometer. For the first measurement, the patient was placed in quadriceps relaxed in supine position. Lower extremity rotation was neutralized using the axial rotation neutralizing apparatus (ARNA). For the second measurement, while the quadriceps was maximally contracted in the same position the midpoint of the patella was re-marked and then the measurement was repeated. The Q angle measurements taken in quadriceps relaxed and MVC were 18.44° ± 2.80° and 16.41° ± 2.83°, respectively (p = .00). It was determined that there was no correlation between age, height, weight, body mass index (BMI), leg length and Q angle measurements of the participants (p > .05). In conclusion, this measurement method offered measurement estimations that verified the effect of the quadriceps on the Q angle in patients with PFPS. The prevention of axial rotation in the leg and the use of an extendable fixed-arm goniometer contribute to the standardization of Q angle measurement.

Objective The present study determined the pattern of presentation and severity of patellar tendinopathy (PT) and its relationship with selected biomechanical variables in elite athletes. Methods The study involved 98 elite Nigerian basketball and volleyball players aged between 18 and 35 years. Clinical tests and ultrasound imaging were used to divide the participants into symptomatic and asymptomatic groups. Standard procedures were used to assess the quadriceps angle (Q-angle), tibial torsion, ankle dorsiflexion, hamstring flexibility, and foot posture. The Victorian Institute of Sport Assessment Questionnaire, Patellar Tendon (VISA-P) was used to assess the severity of the symptoms. The statistical analysis was performed using analysis of variance (ANOVA), and a post hoc analysis and Pearson correlation with significance level set at p < 0.05 were also performed. Results Significantly lower sit-and-reach scores ( p = 0.01), increased foot posture index score ( p = 0.01) and reduced ankle dorsiflexion range ( p = 0.03) were found in participants of both sexes with symptomatic PT. Higher Q-angles ( p = 0.02) in males and tibial torsion angles ( p = 0.001) in females were also found in the symptomatic PT groups. Symptom severity was significantly higher in the group with clinical symptoms only ( p = 0.042), and it was significantly correlated with hamstring flexibility in both males (r = -0.618) and females (r = -0.664). Conclusion Reduced hamstring flexibility, increased foot pronation and reduced ankle dorsiflexion range were significant in participants with symptomatic PT.

Introduction: The Q-angle is defined as the acute angle formed by the vectors for combined pull of the quadriceps femoris muscle and the patellar tendon. It has a great clinical and biomechanics significance. The present study was planned so as to study Q-angle in healthy young adult individuals and establish its relationship with different anthropometric parameters. Material and methods:The subjects for the study were normal healthy young adult medical students from A.C.S. medical college, Chennai, India.148 subjects (80 girls and 68 boys)were studied. Males and females between the age of 18-20 years were included in the study. Q-angle was measured using standard goniometer. The Q-angle in degree was measured on both sides. These Q-angles were correlated with various anthropometric parameters (height, weight, BMI, WHR, pelvic width, femur length).Results: The mean Q-angle in males on left side was 8.1+1.83and that on right side was 8.6 +2.20The mean Q-angle in females on left side was 8.8+ 2.33 and that on right side was 8.9 + 2.52. There were no significant bilateral differences The Q-angle (Left) and Q-angle (Right) are significantly correlated with Weight (Kg), Waist (cm) and Hip (cm). However, these angles are not correlated with Weight for height or Waist for Hip ratio, pelvic width and femur length in females.In case of males, height, BMI, Pelvic width measurements were found to be correlated with Q-Left and Q-Right angle. Femur length was correlated only with Q-Left angle while there was no correlation with Q-Right angle. Conclusion:No significant differences in both gender was also noted, No significant bilateral differences were seen. According to our study Q-angle does not vary with the age. Several anthropometric measures on correlating with Q-angle signify that irrespective of gender higher Q-angles are seen with those having high BMI, increased waist and hip-circumference.
