Normal Palmar Anatomy and Variations That Impact Median Nerve Decompression

Abstract: Annually, carpal tunnel release is one of the most commonly executed orthopaedic procedures. Despite the frequency of the procedure, complications may occur as a result of anatomic variations. Understanding both normal and variant anatomy, including anomalies in neural, vascular, tendinous, and muscular structures about the carpal tunnel, is fundamental to achieving both safe and efficacious surgery. Reviewing and aggregating this information reveals certain principles that may lead to the safest possible surg… Show more

“…Motor deficits include weakness in flexion of the radial half of the digits and of the thumb and weakness in abduction and opposition of the thumb. Furthermore, the presence of an ape hand deformity may be present when attempting to form a fist also known as the benediction sign [23]. Sensory deficits include numbness and tingling in lateral 3.5 digits and their respective nail beds but not in the thenar eminence.…”

“…Carpal tunnel syndrome (CTS), the most common compression neuropathy, results from median nerve entrapment in the carpal tunnel, accounting for about 90% of all such disorders. 1 2 The clinically confirmed CTS prevalence was 9.6% in the general population of China, 3 with a yearly incidence rate of 2.76‰, and women is more susceptible than men. 2 4 CTS has significant impact on daily life and ability to work, 5 and causes great burden on social economy, with an annual associated costs estimated at US$13 billion.…”
