Non‐smoking‐related chronic obstructive pulmonary disease: A neglected entity?

Abstract: Chronic obstructive pulmonary disease (COPD) is an increasing cause of morbidity and mortality worldwide, and it has been strongly correlated to tobacco smoking. While a number of studies have concentrated on smokers only, recent published data demonstrate that at least one fourth of patients with COPD are nonsmokers, and that the burden of COPD in non-smokers is also higher than previously believed. Risk factors of COPD in non-smokers may include genetic factors, long-standing asthma, outdoor air pollution (f… Show more

“…However, previous studies mainly focused on COPD in smokers, studies of non-smoking COPD were limited [7]. The mechanism of COPD, especially in never-smokers, and the biomarkers and therapeutic targets of this disease at molecular levels have become targets of interest.…”

“…Globally, a noticeable proportion of COPD cases are known to be nonsmokers, ranging from 22.9% in UK ( 6 ) to 47% in South Africa ( 5 ). In India specifi cally, 69% of COPD cases were found to be never smokers in a study conducted on 1200 slum dwellers in Pune, Maharashtra ( 7 ).…”

“…Th is in turn has over shadowed the importance of other lifestyle and environmental risk factors ( 5 ). Recent evidences suggest that non-smoking related COPD is higher than previously believed, particularly in the low and middle income countries (LMICs), accounting for 1/4 th to 1/3 rd of all COPD cases ( 6 ).…”

“…(9) Risk factors for COPD in nonsmokers may include genetic factors, longstanding asthma, outdoor air pollution (from traffic and other sources), environmental tobacco smoke exposure, biomass smoke exposure, occupational smoke exposure, diet, recurrent respiratory infection in early childhood and tuberculosis. (10) Nonsmoking COPD is a neglected entity typically excluded in clinical trials, with a lack of characterisation of its profile. …”
